Mr-Proadm Elevation Upon Icu Admission Predicts the Outcome of Septic Patients and is Correlated with Upcoming Fluid Overload.
2017
IF 3.113; International audience; Background: Among septic patients admitted to the intensive care unit (ICU), early recognition of those with the highest risk of death is of paramount importance. We evaluated the prognostic value of Procalcitonin (PCT), mid regional-proadrenomedullin (MR-proADM), copeptine and CT-proendothelin 1 (CT-ProET 1) concentrations. Methods: This was a prospective cohort study, which included 173 septic patient admitted to one ICU. Blood samples for biomarker measure-ments were obtained upon admission and on day 5. The predictive value of each biomarker regarding the risk of death at day 28 was assessed. The fluid balance was evaluated from admission to day 5. Resu…
Relaxation for a Class of Control Systems with Unilateral Constraints
2019
We consider a nonlinear control system involving a maximal monotone map and with a priori feedback. We assume that the control constraint multifunction $U(t,x)$ is nonconvex valued and only lsc in the $x \in \mathbb{R}^{N}$ variable. Using the Q-regularization (in the sense of Cesari) of $U(t,\cdot )$, we introduce a relaxed system. We show that this relaxation process is admissible.
Determinants of between-hospital variations in outcomes for patients admitted with COPD exacerbations: findings from a nationwide clinical audit (AUD…
2015
Background Previous studies have demonstrated significant variability in the processes of care and outcomes of chronic obstructive pulmonary disease (COPD) exacerbations. The AUDIPOC is a Spanish nationwide clinical audit that identified large between-hospital variations in care and clinical outcomes. Here, we test the hypothesis that these variations can be attributed to either patient characteristics, hospital characteristics and/or the so-called hospital-clustering effect, which indicates that patients with similar characteristics may experience different processes of care and outcomes depending on the hospital to which they are admitted. Methods A clinical audit of 5178 COPD patients co…
[Clinical audit of patients admitted to hospital in Spain due to exacerbation of COPD (AUDIPOC study): method and organisation].
2010
There is little information regarding the clinical management of hospital inpatients diagnosed with exacerbation of Chronic Obstructive Pulmonary Disease (COPD). AUDIPOC is a clinical audit dealing with the clinical management of COPD in Spain. To examine the adequacy and validity of the instruments used to measure the variables proposed by AUDIPOC Spain (Preliminary Study) and to verify the viability of AUDIPOC in a complex environment with hospitals of different sizes, resources, and organizational layout (Pilot Study). The Preliminary Study took place in 4 hospitals and studied 213 cases. The Pilot Study took place in 30 hospitals of 6 Autonomous Communities (i.e. Regions) and studied 1203…
Teletraffic Engineering for Direct Load Control in Smart Grids
2018
International audience; The traditional paradigm for power grid operation is to continuously adapt energy production to demand. This paradigm is challenged by the increasing penetration of renewable sources, that are more variable and less predictable. An alternative approach is the direct load control of some inherently flexible electric loads to shape the demand. Direct control of deferrable loads presents analogies with flow admission control in telecommunication networks: a request for network resources (bandwidth or energy) can be delayed on the basis of the current network status in order to guarantee some performance metrics. In this paper we go beyond such an analogy, showing that u…
Endpoint Admission Control over Assured Forwarding PHBs and Its Performance over RED Implementations
2001
The Assured Forwarding Per Hop Behavior (AF PHB) has been devised by the IETF Differentiated Services (DiffServ) working group to provide drop level differentiation. The intent of AF is to support services with different loss requirements, but with no strict delay and jitter guarantees. Another suggested use of AF is to provide differentiated support for traffic conforming to an edge conditioning/policing scheme with respect to nonconforming traffic. Scope of this paper is twofold. First, we show that, quite surprisingly, a standard AF PHB class is semantically capable of supporting per flow admission control. This is obtained by adopting the AF PHB as core routers forwarding mechanism in c…
PCP: An End-to end Measurement Based Call Admission Control for Real-Time Services Over IP Networks
2000
Distributed end-to-end measurement based connection admission control mechanisms have been recently proposed. The goal of these schemes is to provide tight QoScon trol on a per connection basis by means of measurements taken by the edge nodes and priority based forwarding procedure at internal nodes. Since the additional flows handling procedures are implemented at the border routers and the forwarding mechanisms are for flows aggregates only, the approach is fully scalable and compatible with the IETF Differentiated Service proposal. The aim of this paper is to propose specific schemes and to investigate the advantages and limits of the approach by analyzing the basic mechanisms and evalua…
Severe iatrogenic admissions in a coronary care unit:
2013
Introduction: Iatrogenic complications are defined as adverse drug reactions or complications induced by non drug interventions, such as cardiac devices or stimulation techniques. Iatrogenic complications occurring during hospital stay are known to be associated with increased hospital length of stay and mortality. Only few data are available on iatrogenic as cause of hospital admission, particularly in coronary care unit. In patient admitted in coronary care unit for iatrogenic, we aimed a) to analyse their prevalence, type and characteristics, b) to analyse their in-hospital length of stay and mortality and c) to evaluate the predictive factors of severity and mortality. Methods: From 1st…
Impact of a stay in the intensive care unit on the preparation of Advance Directives: Descriptive, exploratory, qualitative study.
2017
Abstract Background Our objective was to assess, through a qualitative, exploratory study, the thought processes of patients regarding the formulation of advance directives (AD) after a stay in the ICU. Methods The study was conducted from May to July 2016 using telephone interviews performed by four senior ICU physicians. Inclusion criteria were: patients discharged from ICU to home > 3 months earlier. Semi-directive interviews with patients focused on 5 main points surrounding AD. Results In total, among 159 eligible patients, data from 94 (59%) were available for analysis. Among all those interviewed, 83.5% had never heard of “advance directives”. Only 2% had executed AD before ICU admis…
