Healthy aging in the context of the Mediterranean diet–health-environment trilemma

Abstract Successful aging results from a lifetime of interaction between a range of factors, including those that are inherited (age, genetics), and those related to lifestyle (diet, exercise). In this brief communication, we examine the role of the Mediterranean-style diet in human health. Diet is one of the major pillars of healthy aging, and accumulating evidence supports the health benefits of the Mediterranean diet. We also discuss the lifelong effect of exposure to environmental pollution. Thus, there is an intricate relationship between health, diet and environment, which together represent a trilemma that must be addressed with a holistic, life-course, population-level approach.

La "Donna di Ostuni", the Lady from Ostuni (fortified medieval city, on the southern Italian Adriatic coast) is the skeleton of "the human most ancient mother" ever found by paleoanthropologists, grave dated of 28,000 years BP. It concerns a 20-years-old woman buried with her baby in her womb estimated at 8 months gestation. To date, the cause of the maternal-fetal deaths is qualified of unknown origin. Hyperglycemia is detected and integrated by the mediobasal hypothalamus (MBH) which, in turn, inhibits food intake and triggers insulin secretion. The MBH houses specialized glucose-sensitive (GS) neurons, which directly or indirectly modulate their electrical activity in response to changes in glucose level. In a first study, we hypothesized that indirect detection of glucose by MBH GS neurons involves the secretion of endozepine by astrocytes, a gliotransmitter known to inhibit food intake in response to hyperglycemia.
