Does the Artery-first Approach Improve the Rate of R0 Resection in Pancreatoduodenectomy? A Multicenter, Randomized, Controlled Trial

[Objective]: To compare the rates of R0 resection in pancreatoduodenectomy (PD) for pancreatic and periampullary malignant tumors by means of standard (ST-PD) versus artery-first approach (AFA-PD). [Background]: Standardized histological examination of PD specimens has shown that most pancreatic resections thought to be R0 resections are R1. “Artery-first approach” is a surgical technique characterized by meticulous dissection of arterial planes and clearing of retropancreatic tissue in an attempt to achieve a higher rate of R0. Computed tomography is generally considered to be the most reliable and, indeed, the definitive diagnostic technique for demonstration of brain tumors. Approximately 98% of intracra-nial lesions are detected with CT. However, extremely high sensitivity is not the only advantage of CT in diagnosis of intracranial tumors. Accuracy in determining histological type, visualization of anatomical relations to important brain structures, knowledge of the growth pattern, and evidence of space-occupying effects of the tumor are at least as important for the neurosurgeon. Thoracic dumbbell spinal chordoma mimicking a schwannoma

Background: Epidural dumbbell-shaped chordomas are localized slow growing, and malignant/aggressive neoplasms. Here, we present a 62-year-old male with a T3-T4 dumbbell-shaped chordoma and reviewed the appropriate literature. Case Description: A 62-year-old male presented with a three-month history of thoracic pain. When the thoracolumbar magnetic resonance (MR) showed a T3-T4 dumbbell-shaped intracanalicular/extradural tumor, he underwent tumor removal. Hypokalemic rhabdomyolysis associated with Bartter's syndrome.

Severe potassium deficiency is an uncommon cause of rhabdomyolysis. We recently treated a 45-year-old patient with myalgia, serious generalized weakness, increased serum creatine kinase and myoglobin level as well as excessive hypokalemia. Histological examination of deltoid muscle biopsy showed rhabdomyolysis. After complete recovery of muscle damage by potassium substitution Bartter's syndrome proved to be the cause of initial and persistent hypokalemia.

Nieru audzēju klīniski morfoloģiskais raksturojums

Saslimstība ar ļaundabīgajiem nieru audzējiem palielinās katru gadu, visizteiktāk tas vērojams pasaules attīstītajās valstīs, kā arī Latvijā. Nieru šūnu karcinoma ir viens no visbiežākajiem ļaundabīgajiem nieru audzējiem. Agrīnajās stadijās nieru šūnu karcinomai nav raksturīga kāda konkrēta simptomātika un lielākajā daļā gadījumu, tā tiek diagnosticēta netīši- veicot izmeklējumus kādas citas saslimšanas gadījumā. Pēc tam, kad audzējs ir ticis diagnosticēts, ir būtiski izvērtēt pacienta vispārējo stāvokli un riska faktorus, no kā ir atkarīga pacientu kopējā dzīvildze un recidīva risks.
