Biology of the sauropod dinosaurs: the evolution of gigantism

The herbivorous sauropod dinosaurs of the Jurassic and Cretaceous periods were the largest terrestrial animals ever, surpassing the largest herbivorous mammals by an order of magnitude in body mass. Several evolutionary lineages among Sauropoda produced giants with body masses in excess of 50 metric tonnes by conservative estimates. With body mass increase driven by the selective advantages of large body size, animal lineages will increase in body size until they reach the limit determined by the interplay of bauplan, biology, and resource availability. Background Trunk forward bending is one of the most common activities in daily living; it is a two-part movement involving the lumbar spine flexion and pelvis rotation at hip joint. The pattern of movement during forward bending was defined as the relative contribution of the lumbar spine to pelvis motion, and was expressed as the ratio between the ranges of lumbar spine motion to pelvis motion (L/P), which, calculated at certain degrees of trunk flexion during the entire movement, provides the lumbo-pelvic rhythm. Pēcoperācijas analgēzijas līmeņa izvērtēšana pēc n. ischiadicus bloka ar multimodālās analgēzijas protokolu apakšstilba lūzuma gadījumā

Neskatoties uz mūsdienu medicīnas attīstību, augsts pēcoperācijas sāpju līmenis vēl aizvien ir aktuāla problēma, īpaši traumatoloģijā. Apakšstilba lūzuma rekonstrukcijas operācijās kā pamata anestēzijas metode tiek lietota spinālā anestēzija (SA). Tiek meklētas papildus iespējas, kuras iekļaut multimodālās analgēzijas shēmā, lai efektīvi kontrolētu sāpes. N. ischiadicus bloks (NIB) varētu būt efektīvs perifēriskās nervu blokādes veids, kas samazinātu pēcoperācijas sāpju līmeni. Pētījuma aktualitāte: Apakšstilba vai pēdas pēcoperācijas sāpju mazināšanai multimodālas analgēzijas ietvaros tiek veikta n.ischiadicus blokāde. Tā kā lokālo anestētiķu darbības ilgums ir samērā īss, tiek veikti pētījumi par deksametazonu kā adjuvantu blokādes ilguma pagarināšanai. Mēs vēlamies Latvijā izpētīt deksametazona iedarbību uz n.ischiadicus bloka analgētiskās darbības ilgumu. Mērķis: Samazināt sāpes pacientiem pēc apakšstilba vai pēdas operācijām, pagarinot n.ischiadicus bloka analgētisko darbību, izmantojot deksametazonu kā adjuvantu.
