Successful Foley catheter tamponade of an epigastric vessel trocar injury in a toddler – A case report

Highlights • Epigastric vessel injury is a well recognized complication during laparoscopic trocar placement. • Precautionary measures to prevent trocar induced injuries should be implemented to lower the incidence of this complication. • If this complication is encountered, tamponade by Foley catheter placement is an effective measure to provide hemostasis. • Hemorrhage control using a Foley catheter is feasible even in small children and toddlers.

Highlights • A multidisciplinary team is mandatory for the correct management of hemorrhagic GIST and its complications. • There is a well-known association between type 1 Neurofibromatosis and GIST. • Type 1 Neurofibromatosis-GIST and sporadic GIST have different behaviour. • In case of localised and resectable GIST surgical treatment is the mainstay. • Laparoscopic approach, if performed correctly, is safe and effective with better short-term outcomes then open surgery.

Five women and one man (average age 62.5 [20-64] years) with symptomatic intraabdominal cysts at different sites (liver: n = 3; spleen: n = 2; mesentery: n = 1) had them removed by laparoscopic surgery. Broad fenestration was performed with the hepatic and splenic cysts, while the mesenteric cyst was enucleated from the mesentery. There were no intra- or postoperative complications. Median duration of operation was 60 (45-125) min, postoperative hospital stay was 5 (3-6) days. The aim of this study was to compare the impact of induced bleeding complication training with regular training on a virtual reality laparoscopic (VRL) simulator. Although bleeding complications occur rarely during laparoscopic surgery, they usually arise without warning and may have severe consequences for the patient because complication management training is not currently widespread. Highlights • Patients with lack of factor VIIa, suffering from abnormal uterine bleedings can be treated with laparoscopic hysterectomy. • Technique using bipolar coagulation and non-absorbable clips is safe and not time consuming. • Pre-operative loading with recombinant factor VIIa must be followed by prolonged supplementation in post-operative period. • We present suggested dosage and length of factor VIIa supplementation based on literature review and own experience.

Melatonin, the hormone of circadian rhythm regulation, is involved in the modulation of mitochondrial activity through its antioxidant and anti-inflammatory properties. Alteration of circadian rhythms such as sleep is related to obesity and metabolic pathogenesis in adulthood, but studies during childhood are scarce. The present study investigated the association of melatonin with metabolic and inflammatory markers in children with (n = 113) and without obesity (n = 117).
