Left Spigelian Hernia in Super-Obese Patient: A Case-Report.

Abstract: We report a case in which a giant Spigelian hernia presented with severe, persistent abdominal pain in a 62- year-old super-obese woman with a history of total abdominal hysterectomy-bilateral salpingo-oopherectomy at the age of 40 years for benign neoplasia, who later underwent open hernia repair (maximal diameter of the hernia sac: 21 cm) because of the development of compartment syndrome. Afterward, although a large incarcerated hernia disease was found, the sac removal was technically difficult and an open procedure was carried out, with double-mesh placement. Background Obesity is associated with high cardiovascular risk. Postprandial lipidaemia has been associated with cardiovascular disease risk. Our aim was to identify whether anthropometric parameters, insulin resistance (IR) and/or fasting plasma triglycerides may determine postprandial changes in lipoprotein concentrations in abdominal and morbid obese subjects. Methods We have studied 20 non-diabetic, normolipidaemic subjects with abdominal obesity, 20 morbid obese subjects and 20 healthy individuals, that have similar age and gender. Obesity is characterized by chronic low grade of systemic inflammation that develops in response to nutrient excess and plays a key role in the pathogenesis of insulin resistance. It is characterized by macrophage infiltration into adipose tissue (AT) and abnormal cytokine production. These factors damage the metabolic homeostasis leading to alteration in the insulin signaling in specific tissues and organs such as AT and liver. Because adipose tissue is a complex endocrine organ that secretes many substances with profound effects on metabolism and the cardiovascular (CV) system, most obese subjects have an increased CV risk. However, 20% of obese subjects are metabolically healthy, and many studies suggest that fat distribution, in particular abdominal fat excess, is the most important factor that determines a secretion of adipose products that may increase CV risk.

Impaired lipid metabolism is a common risk factor underlying several metabolic diseases such as metabolic syndrome and type 2 diabetes. Branched-chain amino acids (BCAAs) that include valine, leucine and isoleucine have been proven to share a role in lipid metabolism and hence in maintaining metabolic health. Attachment Anxiety and Attachment Avoidance: Members’ Attachment Fit with Their Group and Group Relationships

We compared each individual group member’s attachment fit with the attachment of the other group members. Participants were 201 members of 20 therapy groups who sought treatment because they were overweight or obese. Group members completed the Attachment Style Questionnaire before commencing treatment and the Group Questionnaire at three points during group treatment.
