Implantation of Gore-Tex chordae on aortic valve leaflet to treat prolapse using "the chordae technique": surgical aspects and clinical results.

Background Repair of prolapsed aortic valve leaflets has been considered a challenging technique for cardiac surgeons. In this paper we describe our surgical approach, "the chordae technique." It consists of the correction of aortic cusp prolapse by shortening the free margin length and of an adjustment of the leaflets coaptation height by anchoring the prolapsing cusp to the aortic wall at the sinotubular junction level. Methods Between February 2003 and December 2006, 26 patients with one or more prolapsed aortic leaflets underwent surgical repair using the new approach. The mean age of patients was 55 ± 10 years. Revaskularisation mit der A. gastroepiploica dextra — Bewertung der Indikation und des perioperativen Risikos

Bei 35 Patienten (mittleres Alter 64,0 Jahre (±10,4 Jahre) wurde bei fehlender oder unbrauchbarer V. saphena magna neben der einoder beidseitigen A. thoracica interna (ITA) die rechte A. gastroepiploica (GEA) zur Revaskularisation der rechten Koronararterie (RCA) verwendet. Ausgeschlossen wurden Patienten mit erheblicher Adipositas (Broca-Index >20%) sowie Patienten mit abdominellen Voroperationen. In einem Fall konnte die GEA wegen Plaquebildung nicht verwendet werden. Bei 23 Patienten wurde die GEA mit dem R. interventricularis posterior (RIVP), bei 11 Patienten mit der RCA vor bzw. in Hohe der Crux anastomosiert.

Remodeling der Aorta als rekonstruktive Maßnahme für Aneurysmen der Aorta aszendens

Eine weniger haufig gebrauchte Methode zur operativen Therapie des Aszendens-Aneurysmas ist die plastische Rekonstruktion im Sinne einer Reduktionsplastik mit anschliesender Ummantelung der nativen Aorta. Der Vorteil liegt in der vollstandigen Erhaltung der Endothelzellschicht der Aorta und der Beibehaltung der dynamischen Funktion der Sinus Valsalvae.¶ Von 1987–1998 wurden 20 Patienten mit einem Aneurysma verum nach der o.g. Methode operiert. In 9 Fallen (Gruppe I – mittleres Patientenalter 72 Jahre; Spannweite 64–78 Jahre) beschrankte sich der Eingriff auf die plastische Rekonstruktion der Aorta und konsekutive Ummantelung. Primäres Leiomyosarkom der Pulmonalarterie: Inzidenz, Diagnostik und Therapie

Leiomyosarkome des Herzens und der grosen Gefase sind sehr seltene hochmaligne Tumoren, die meist rasch zum Tode fuhren.¶ Wir berichten uber einen 31-jahrigen Patienten mit einem von der Pulmonalklappe ausgehenden Leiomyosarkom, das beide Pulmonalarterien und den Ausflusstrakt des rechten Ventrikels miteinbezog. Infolge der raschen Diagnosestellung und nachfolgenden radikalen Resektion ist der Patient nach mehr als 5 Jahren weiterhin tumorfrei. Nur die fruhzeitige Diagnose und komplette chirurgische Excision konnen die Uberlebenschance fur Patienten mit Pulmonalissarkomen erhohen.

Schwere sternale Wundkomplikationen nach kardiochirurgischen Eingriffen im Erwachsenenalter

Schwere postoperative Wundheilungsstorungen bis hin zur kompletten Sternumdehiszenz sind gravierende Komplikationen nach medianer Sternotomie in der Herz-Thoraxchirurgie. Sie fuhren zu einer erhohten Morbiditat- und Mortalitatsrate und verursachen eine sehr viel langere Hospitalisation der Patienten als bei normaler Wundheilung. Fur das Krankenhaus entstehen daraus zusatzliche Kosten. Die vorliegende Ubersichtsarbeit beschaftigt sich mit den schweren sternalen Wundheilungsstorungen und ihrer Atiologie, Symptomen, Diagnose, pradisponierenden Risikofaktoren und Behandlungsstrategien und berucksichtigt dabei die weltweit jungste Literatur.

Single-staged laryngotracheal reconstruction for idiopathic tracheal stenosis.

Background This study retrospectively evaluated the results of surgically treated idiopathic tracheal stenosis. Methods Of the 220 patients surgically treated for idiopathic subglottic and tracheal stenosis in the participating hospitals, we reviewed the surgical records of all patients with idiopathic tracheal stenosis. This subgroup required resection of all of the involved mucosa but frequently had undergone more conservative treatments that damaged the tracheal mucosa and cartilage and complicated the definitive surgical treatment. Validation and update of the thoracic surgery scoring system (Thoracoscore) risk model.

Abstract OBJECTIVES The performance of prediction models tends to deteriorate over time. The purpose of this study was to update the Thoracoscore risk prediction model with recent data from the Epithor nationwide thoracic surgery database. METHODS From January 2016 to December 2017, a total of 56 279 patients were operated on for mediastinal, pleural, chest wall or lung disease. We used 3 recommended methods to update the Thoracoscore prediction model and then proceeded to develop a new risk model. Effects of the novel polymer gel LeGoo on human internal thoracic arteries.

Purpose Established hemostatic devices can injure vessel wall integrity. LeGoo (Pluromed, Woburn, MA), a novel poloxamer gel with reverse thermosensitive properties, is a new device for temporary occlusion of blood vessels. The present study investigated the effects of LeGoo on vascular function and morphology. A particular phenotype of ascending aorta aneurysms as precursor of type A aortic dissection.

Objectives: We aimed to identify a phenotype of ascending thoracic aortic aneurysm (TAA), which, more than others, evolves into type A dissection (TAD). Methods: Aortic specimens were obtained from patients undergoing surgical repair of TAA and TAD (108 and 26, respectively).
