Use of neural networks for dosage individualisation of erythropoietin in patients with secondary anemia to chronic renal failure.

The external administration of recombinant human erythropoietin is the chosen treatment for those patients with secondary anemia due to chronic renal failure undergoing periodic hemodialysis. The goal is to carry out an individualised prediction of the erythropoietin dosage to be administered. It is justified because of the high cost of this medication, its secondary effects and the phenomenon of potential resistance which some individuals suffer. One hundred and ten patients were included in this study and several factors were collected in order to develop the neural models. Since the results obtained were excellent, an easy-to-use decision-aid computer application was implemented.

Background: The NeoGuide Endoscopy System (NES) utilizes a fully articulated, computer-controlled insertion tube that allows proximal segments of the colonoscope to follow the path taken by the tip as it is manually advanced through the colon. Most colon cancers develop from nonmalignant colonic adenomas or polyps over a comparatively long time period ranging between 24 and 60 months. Reflecting this adenomatous pathogenesis of most colorectal cancers, polyp screening with subsequent polypectomy is used to constitute an effective approach for decreasing the incidence of this malignant tumor. Sonography and CT were used pre-operatively for lymph node staging in patients with head and neck malignancies. The accuracy of the imaging methods surpassed that of palpation (palpation 85%, CT 85%, sonography 90%). Sensitivity was significantly increased from 74% (palpation) to 84% (CT) and 90% (sonography), ie. there was a reduction in false negative findings. Size of lymph nodes was not closely correlated with metastatic involvement. Reactively enlarged lymph nodes were more easily defined by CT and sonography than by palpation. This reduced the specificity of sonography (90%) and of CT (86%) compared to palpation (94%).

Enhanced imaging technologies are of special importance in patients with inflammatory bowel diseases. Patients with long-lasting colitis face an increased cancer risk and neoplastic changes often evolve as subtle, multifocal, and flat lesions. Enhanced endoscopic imaging technologies enable better visualization of the mucosal architecture. These new available information can be used to better detect and characterize colorectal lesions.
