Transient Pulmonary Infiltrates during Treatment with Anti-Thymocyte Globulin

We report the case of a 54-year-old woman with aplastic anemia, who developed transient pulmonary infiltrates following intravenous infusion of rabbit antithymocyte globulin (ATG) for 3 days. There was no other explanation than the infusion of ATG for the infiltrates. Rechallenge with ATG induced the recurrence of opacities on the chest radiograph. Although rarely involved with only 4 previous reports, ATG should be included in the list of drugs capable of inducing pulmonary infiltrates.

the lack of comparative studies and of multicenter trials still makes the treatment of the lung injured drowned patient a challenge for intensivists. Although the outcome of drowned patients is related to the potential neurological sequalae, AHRF remains the main concern. Ventilation guidelines for drowning patients are mainly adapted from AHRF patients and may not reflect the needs of this particular population. Our result confirms that early pronation, in cases of lack of improvement of oxygenation, might be the correct strategy.14 In addition, HFNO and awake prone position might have a role in avoiding reintubation while maintaining adequate pulmonary gas exchanges.

Background: Panallergens are proteins that take part in key processes of organisms and, therefore, are ubiquitously distributed with highly conserved sequences and structures. One class of these panallergens is composed of the tropomyosins. The highly heat-stable tropomyosins comprise the major allergens in crustaceans and mollusks, which make them important food allergens in exposed populations.
