Resume La loi no 2016-87 du 2 fevrier 2016 creant de nouveaux droits en faveur des malades et des personnes en fin de vie renforce les droits precedents et introduit un droit nouveau. Le statut de la personne de confiance est renforce. Les directives anticipees deviennent contraignantes mais ne sont pas opposables : elles s’imposent au medecin sauf dans trois cas : une urgence vitale, un caractere manifestement inapproprie ou une redaction non conforme a la situation medicale du patient. Pain not responsive to morphine is often problematic. Animal and clinical studies have suggested that N-methyl-d-aspartate (NMDA) antagonists, such as ketamine, may be effective in improving opioid analgesia in difficult pain syndromes, such as neuropathic pain. A slow bolus of subhypnotic doses of ketamine (0.25 mg/kg or 0.50 mg/kg) was given to 10 cancer patients whose pain was unrelieved by morphine in a randomized, double-blind, crossover, double-dose study. In recent years, there has been a considerable increase in the number of image-guided interventional procedures performed for the management of acute and chronic pain. Concomitantly, there has also been an increase in the complication rate related to these procedures. The aim of this narrative review is to summarize the primary complications associated with commonly performed image-guided (fluoroscopic- or ultrasound-guided) interventional procedures. We conclude that although complications from interventional pain procedures can be mitigated to a certain degree, they cannot be eliminated altogether. Summary Respiratory failure (RF) is defined as a disturbance in gas exchange in the respiratory system which produces in arterial BGA a PaO 2 2 > 50 mmHg (hypercapnia). However hypoxaemic normocapnic (or hypocapnic) RF due to the failure in gas exchange is very common and should be separated from mechanical RF. Respiratory failure (hypercapnic) with or without hypoxaemia related to a failure in the respiratory pump. This review is focused on the pathophysiology of the mechanical RF less well known amongst anaesthesiologists.
