Chronic kidney disease in patients with obstructive sleep apnea. A narrative review.

2018

Prevalence of both chronic kidney disease (CKD) and obstructive sleep apnea (OSA) is continuously increasing. Moreover, the prevalence of OSA increases as kidney function declines and is higher among patients with end-stage renal disease (ESRD). In addition, OSA is recognized as a potential nontraditional risk factor for development and progression of CKD. Continuous positive airway pressure (CPAP) plays a pivotal role in the management of OSA, eliminating patients' symptoms and improving their quality of life. Recent studies suggested that CPAP treatment may have beneficial effects on kidney function among patients with OSA. This narrative review summarizes the existing knowledge on the as…

Sleep apnoea and metabolic dysfunction.

2013

Obstructive sleep apnoea (OSA) is a highly prevalent condition often associated with central obesity. In the past few years, several studies have analysed the potential independent contribution of OSA to the pathogenesis of metabolic abnormalities, including type 2 diabetes, the metabolic syndrome and non-alcoholic fatty liver disease. New perspectives in OSA patient care have been opened by the promotion of lifestyle interventions, such as diet and exercise programmes that could improve both OSA and the metabolic profile. The rich clinical literature on this subject, together with the growing amount of data on pathophysiological mechanisms provided by animal studies using the chronic inter…

Dynamic determination of oxygenation and lung compliance in murine pneumonectomy.

2011

Thoracic surgical procedures in mice have been applied to a wide range of investigations, but little is known about the murine physiologic response to pulmonary surgery. Using continuous arterial oximetry monitoring and the FlexiVent murine ventilator, the authors investigated the effect of anesthesia and pneumonectomy on mouse oxygen saturation and lung mechanics. Sedation resulted in a dose-dependent decline of oxygen saturation that ranged from 55% to 82%. Oxygen saturation was restored by mechanical ventilation with increased rate and tidal volumes. In the mouse strain studied, optimal ventilatory rates were a rate of 200/minute and a tidal volume of 10 mL/kg. Sustained inflation pressu…

Postoperative continuous positive airway pressure to prevent pneumonia, re-intubation, and death after major abdominal surgery (PRISM): a multicentre…

2021

BackgroundRespiratory complications are an important cause of postoperative morbidity. We aimed to investigate whether continuous positive airway pressure (CPAP) administered immediately after major abdominal surgery could prevent postoperative morbidity.MethodsPRISM was an open-label, randomised, phase 3 trial done at 70 hospitals across six countries. Patients aged 50 years or older who were undergoing elective major open abdominal surgery were randomly assigned (1:1) to receive CPAP within 4 h of the end of surgery or usual postoperative care. Patients were randomly assigned using a computer-generated minimisation algorithm with inbuilt concealment. The primary outcome was a composite of…

The right interface for the right patient in noninvasive ventilation: a systematic review

2022

Introduction Research in the field of noninvasive ventilation (NIV) has contributed to the development of new NIV interfaces. However, interface tolerance plays a crucial role in determining the beneficial effects of NIV therapy. Areas covered This systematic review explores the most significant scientific research on NIV interfaces, with a focus on the potential impact that their design might have on treatment adherence and clinical outcomes. The rationale on the choice of the right interface among the wide variety of devices that are currently available is discussed here. Expert opinion The paradigm 'The right mask for the right patient' seems to be difficult to achieve in real life. Rang…

Parameter dependence for the positive solutions of nonlinear, nonhomogeneous Robin problems

2020

We consider a parametric nonlinear Robin problem driven by a nonlinear nonhomogeneous differential operator plus an indefinite potential. The reaction term is $$(p-1)$$-superlinear but need not satisfy the usual Ambrosetti–Rabinowitz condition. We look for positive solutions and prove a bifurcation-type result for the set of positive solutions as the parameter $$\lambda >0$$ varies. Also we prove the existence of a minimal positive solution $$u_\lambda ^*$$ and determine the monotonicity and continuity properties of the map $$\lambda \rightarrow u_\lambda ^*$$.

A note on homoclinic solutions of (p,q)-Laplacian difference equations

2019

We prove the existence of at least two positive homoclinic solutions for a discrete boundary value problem of equations driven by the (p,q) -Laplace operator. The properties of the nonlinearity ensure that the energy functional, corresponding to the problem, satisfies a mountain pass geometry and a Palais–Smale compactness condition.

An Introduction to Hodge Structures

2015

We begin by introducing the concept of a Hodge structure and give some of its basic properties, including the Hodge and Lefschetz decompositions. We then define the period map, which relates families of Kahler manifolds to the families of Hodge structures defined on their cohomology, and discuss its properties. This will lead us to the more general definition of a variation of Hodge structure and the Gauss-Manin connection. We then review the basics about mixed Hodge structures with a view towards degenerations of Hodge structures; including the canonical extension of a vector bundle with connection, Schmid’s limiting mixed Hodge structure and Steenbrink’s work in the geometric setting. Fin…

Completely positive invariant conjugate-bilinear maps on partial *-algebras

2007

The notion of completely positive invariant conjugate-bilinear map in a partial *-algebra is introduced and a generalized Stinespring theorem is proven. Applications to the existence of integrable extensions of *-representations of commutative, locally convex quasi*-algebras are also discussed.

Multifunctions determined by integrable functions

2019

Integral properties of multifunctions determined by vector valued functions are presented. Such multifunctions quite often serve as examples and counterexamples. In particular it can be observed that the properties of being integrable in the sense of Bochner, McShane or Birkhoff can be transferred to the generated multifunction while Henstock integrability does not guarantee it.
