Efficacy of epidural anesthesia for retroperitoneoscopic renal biopsy

Laparoscopic procedures are performed using general anesthesia due to the perceived limitations of regional anesthesia in the upper abdomen and retroperitoneum. We present our initial experience with epidural anesthesia for retroperitoneal laparoscopic renal biopsy.

In recent years, there has been a considerable increase in the number of procedures carried out under regional anesthesia. The techniques used can be associated with a number of complications, which should be understood so that they can be recognized and managed appropriately. The overall incidence of reported complications associated with these techniques is low and therefore, with currently available data, we can only have an approximate idea of their incidence. The objective of this study is to systematically describe the complications that may arise from the use of neuraxial and peripheral regional anesthesia techniques.

Die vorliegende Kasuistik beschreibt den Fall einer Paraplegie nach Anlage eines thorakalen Epiduralkatheters auf dem Boden eines epiduralen Hamatoms im Bereich epiduralen Metastasengewebes. Neben dem genauen Procedere werden die Bedingungen diskutiert, die im Falle einer solchen Komplikation die fruhest mogliche Diagnose und somit die nachfolgende Therapieplanung ermoglichen. Daruber hinaus wird die Notwendigkeit praoperativer Untersuchungen bei geplanter Anlage eines Epiduralkatheters diskutiert.

Zur postoperativen Schmerztherapie bei Kindern mit Operationen an der unteren Korperhalfte bietet sich eine Periduralanasthesie an. Bisher wurde die initiale Pharmakokinetik bei Kindern untersucht, nicht hingegen der langfristige Verlauf. Ziel dieser Untersuchung war die Erhebung von Daten zum zeitlichen Verlauf des Plasma-Bupivacainspiegels, der sauren α1-Glycoprotein- und Albuminkonzentrationen bei Kindern mit kontinuierlicher Periduralanasthesie in der postoperativen Phase. Lumbar sympathetic blocks (LSBs) are commonly performed to treat pain ailments in the lower limbs. LSBs involve injecting local anesthetic around the nerves. The injection is guided by fluoroscopy which is sometimes considered to be insufficiently accurate. The main aim was to analyze the plantar foot skin temperature data acquired while performing LSBs in patients with complex regional pain syndrome (CRPS) affecting the lower limbs.
