Dyspnoe ist einer der haufigsten Grunde fur eine Vorstellung in einer Notaufnahmestation. Die weitere differentialdiagnostische Abklarung erfordert Zeit und verbraucht Ressourcen. Wir untersuchten deshalb, ob die Bestimmung des Brain-natriuretic-peptide (BNP) mittels eines Schnelltests eine zusatzliche Information zur klinischen Einschatzung bietet. Wir untersuchten 100 Patienten, die sich in einer Notaufnahmestation mit dem Leitsymptom „schwere Dyspnoe“ vorstellten. Bei allen Patienten wurde aus dem Uberstand des Blutes, das fur die Routineuntersuchungen gewonnen wurde, mittels eines Schnelltests der Gehalt an BNP bestimmt. Abstract B-type natriuretic peptide (BNP) is a member of a four-natriuretic peptide family that shares a common 17-peptide ring structure. The N-terminal fragment (NT-pro-BNP) is biologically inert, but both are secreted in the plasma in equimolar quantities and both have been evaluated for use in the management of congestive heart failure. BNP and NT-pro-BNP are frequently used in the diagnosis of congestive heart failure and distinguishing between patients with dyspnoea of cardiac or pulmonary origin. Decongestion discriminates risk for one-year mortality in patients with improving renal function in acute heart failure

Aims Improving renal function (IRF) is paradoxically associated with worse outcomes in acute heart failure (AHF), but outcomes may differ based on response to decongestion. BACKGROUND AND AIMS: Plasma levels of NT-pro-BNP, a natriuretic peptide precursor, are raised in the presence of fluid retention of cardiac origin and can be used as markers of cardiac dysfunction. Recent studies showed high levels of NT pro BNP in patients with cirrhosis.
