Tectonic evolution of the Sicilian Thrust System (central Mediterranean)

The Sicilian Thrust System (STS) is a south-verging (Africa-verging) fold-and-thrust belt including a Mesozoic-Paleogene sedimentary sequence. This thrust stack owes its origin to the deformation of pre-orogenic strata deposited in different palaeogeographic domains belonging to passive margins of the African plate. The STS was deformed during the Neogene, following the closure of the Tethys Ocean and the continental collision between the Sardo-Corso Block and the North Africa margins. The thrust pile was detached from the underlying basement during the Miocene-Pleistocene. The Resource Utilization Groups (RUG-III) case-mix classification is becoming an internationally accepted methodology for determining payment for nursing facility and care. In this paper we discuss the key concepts of case-mix reimbursement and several different payment approaches based on RUG-III. In discussing the empirical evidence of case-mix reimbursement, we draw especially on the US experience. Clearly, the success of case-mix reimbursement depends not only on the payment design, but also on the ability to foresee market conditions and the regulatory environment in which the payment system is being implemented. Orphan drugs (ODs) are medicinal products intended for diagnosis, prevention or treatment of life-threatening or very serious diseases affecting less than 5 in 10 000 people in the European Union (EU). These drugs are called “orphans” because the pharmaceutical industry has little interest, under normal market conditions, in developing and marketing products intended for only a small number of patients suffering from very rare conditions. Anti-glomerular basement membrane antibody-mediated glomerulonephritis due to glue sniffing

A 16-year-old girl developed rapidly progressive glomerulonephritis and renal failure. The disease was associated with high titres of antiglomerular basement membrane antibodies in serum, linear deposits of immunoglobulin G and diffuse epithelial crescents on renal biopsy. Past history revealed heavy smoking and deliberate sniffing of Pattex glue, a mixture of hydrocarbons which possibly may affect the structure of glomerular basement membrane. After treatment by repeated plasmapheresis and drug immunosuppression autoantibodies disappeared from serum but renal function was not influenced. Renal damage is a potential hazard for glue sniffing adolescents.

By light and electron microscopical examination it is shown that four structural components can contribute to obsolescent glomeruli: capillary basement membranes, enriched mesangium matrix, “vascular” hyalin and collagen fibers. Each of these components can bring about glomerular damage alone.
