Demographic change in Europe has resulted in an aging population, which presents new challenges for implementing and managing long-term care (LTC) systems. One solution aims to increase the efficiency of LTC by using digital technologies. This article focuses on professionals’ opinions of the use of digital technologies in the administration of Spain’s long-term care law (Law 39/2006, December 14, LAPAD). Resume Introduction Compte tenu des implications de la rehospitalisation a l’echelon individuel et societal, le depistage des personnes âgees (PA) a risque apparait fondamental. Nous avons analyse les facteurs predictifs de rehospitalisation precoce des PA d’au moins 75 ans hospitalisees en medecine. Methodes Etude retrospective monocentrique ayant inclus des PA d’au moins 75 ans hospitalisees en medecine polyvalente via le service d’accueil des urgences. Les patients decedes durant l’hospitalisation, mutes et hospitalises le mois precedent etaient exclus. Zusammenfassung Der Innovationsfonds schafft Anreize zur Entwicklung und Erprobung von Versorgungsinnovationen im Bereich der gesetzlichen Krankenversicherung (GKV) mit dem Ziel einer qualitativen Weiterentwicklung der Versorgung in Deutschland. Uber einen Zeitraum von zunachst vier Jahren (2016-19) werden jahrlich 300 Millionen Euro fur Projekte zu „Neuen Versorgungsformen“ und zur „Versorgungsforschung“ bereitgestellt.
