Clinical disorders responsible for plasma hyperviscosity and skin complications

In this brief review, we have examined some clinical disorders which are associated to an altered hemorheological profile and at times accompanied by skin ulcers. This skin condition may be, in fact, observed in patients with primary plasma hyperviscosity such as multiple myeloma, Waldenstrom macroglobulinemia, cryoglobulinemia, cryofibrinogenemia, dysfibrinogenemia and connective tissue diseases. It must be underlined that the altered hemorheological pattern is not the only responsible for this skin complication but, as it worsens the microcirculatory flow, it contributes to determine the occurrence of the skin ulcers.

Es wird uber den sterischen Verlauf der Oxidation der optisch aktiven, tertiaren Phosphine 3 and 4 mit Dimethylsulfoxid berichtet. Diese Umsetzung verlauft nur in Gegenwart elektrophiler Katalysatoren (Tabelle 1). Je nach Art and Menge des Katalysators zeigt die Stereochemie der Reaktion entweder uberwiegende Inversion oder Retention der Konfiguration am Phosphor an (Tabellen 2, 3); in einigen Fallen wird jedoch fast vollstandige Racemisierung beobachtet (Tabelle3). Abstract Lymphomplasmocytoid/ic immunocytoma (LP-IC), including Waldenstrom’s macro-globulinemia and lymphoplasmacytoid lymphoma according to the Kiel classification (the latter one subsummed as a variant of B-CLL in the WHO classification), is an indolent lymphoma, which is incurable by conventional chemotherapy in the advanced stage of disease. Sulfonation of the N-Boc derivatives of 1,2-aminoalcohols, such as ephedrine, pseudoephedrine, norephedrine, norpseudoephedrine, thiomicamine, and chloramphenicol yields a mixture of the corresponding oxazolidinones with inversion (erythro derivatives) and/or retention of configuration (threo derivatives)at C5. We suggest a competition between two mechanisms: an intramolecular SN2 process initiated by attack of the carbonyl oxygen of the Boc group to the benzylic carbon and the other one through a double SN2 process. Cholesterol reacts under Appel conditions (CBr4/PPh3) to give 3,5-cholestadiene (elimination) and 3β-bromocholest-5-ene (substitution with retention of configuration). Thus, the bromination of cholesterol deviates from the stereochemistry of the standard Appel mechanism due to participation of the Δ5 π-electrons. In contrast, the subsequent azidolysis (NaN3/DMF) of 3β-bromocholest-5-ene proceeds predominantly by Walden inversion (SN2) affording 3α-azidocholest-5-ene. Tutkielman aiheena on vieraan (das Fremde) käsite Bernhard Waldenfelsin filosofiassa. Nykyfilosofian piirissä erilaiset vierauteen ja toiseuteen liittyvät kysymykset näyttäisivät nousevan aina vain keskeisempään osaan aikakautemme akateemisissa diskursseissa. Tätä temaattista tendenssiä ovat vauhdittaneet muun muassa ilmiöt, joiden yhteydessä on alettu käyttää termiä ’globaali’ sekä erilaisilla elämänalueilla lisääntyneet kulttuurien väliset kohtaamiset. Waldenfels on tuonut vieraus- ja toiseusdiskursseihin uusia merkittäviä piirteitä, jotka ovat vielä suhteellisen tuntemattomia.
