Mucormycosis of maxillary sinus in a newly diagnosed case of diabetes mellitus

Rhinocerebral mucormycosis is the third most common fungal infection after aspergillosis and candidiasis. The importance of mucormycosis has grown in recent years, as the number of patients has increased dramatically. Herein, we present a case of a newly diagnosed patient of diabetes mellitus having ulcerated palate and destruction of maxillary sinus evident on computed tomography scan. A biopsy report was suggestive of mucormycosis. Aggressive medical and surgical treatment was done along with the treatment of diabetes mellitus.

Mechanical recanalization in basilar artery occlusion: The ENDOSTROKE study

Objective A study was undertaken to evaluate clinical and procedural factors associated with outcome and recanalization in endovascular stroke treatment (EVT) of basilar artery (BA) occlusion. Methods ENDOSTROKE is an investigator-initiated multicenter registry for patients undergoing EVT. Transcranial Doppler ultrasound (TCD) recording is a non-invasive diagnostic procedure for the evaluation of the cerebral collateral flow in patients, in whom therapeutic ligation or resection of the common and/or internal carotid artery is planned. Patients are first examined under resting conditions, and then under manual compression of the ipsilateral carotid artery. Since January 1989, 31 ENT and neurosurgical patients have been examined. In all patients an immediate decrease in flow velocity in the middle cerebral artery (mca) of about 25% to 90% was recorded. Radiosurgery of Carotid-Cavernous Fistulae

25 cases of carotid-cavernous fistulae (CCF) who underwent radiosurgery with a conventional gamma source from 1977 to 1992 are reported. 22 were low-flow, spontaneous CCFs and 3 were high flow fistulae which had undergone a previous trApplng. The total dose delivered was 30 to 40 Gy. 91% to patients with low-flow CCF cured after radiosurgery in a mean time of 7.5 months, presenting improvent in a mean time of 2.3 months. Only one of the high-flow fistulae was cured. Follow-up period ranged between 14 years and 15 months (mean: 50 months). No recurrence was recorded in any case. Endothelin action on goat cerebral arteries.

Abstract Cumulative application of endothelin-1 (human) markedly constricted goat isolated cerebral arteries in a concentration-dependent manner. Contractile responses were not affected by removal of endothelial cells. Removal of extracellular calcium or addition of the calcium channel blocker nicardipine (10−7 M) failed to abolish responses to endothelin. The results suggest that the endothelium-independent constriction of cerebral arteries produced by endothelin cannot be explained solely by voltage-dependent calcium channels. The contractile responses are likely to be mediated by stimulation of specific receptors for this peptide.

Abstract The generation, propagation and pharmacological properties of low-Mg 2+ -induced epileptiform activity were examined in the intact corticohippocampal formation (CHF) of the newborn (P0–4) mouse in vitro. Multi-site field potential recordings in dentate gyrus (DG), CA3, CA1, entorhinal cortex (EC) and temporal cortex (TC) revealed in 0.2 mM Mg 2+ -containing ACSF a stable pattern of spontaneous epileptiform activity consisting of recurrent ictal-like events (ILEs) and interictal events (IEs). Background Idarucizumab is a monoclonal antibody fragment with high affinity for dabigatran reversing its anticoagulant effects within minutes. Thereby, patients with acute ischemic stroke who are on dabigatran treatment may become eligible for thrombolysis with recombinant tissue-type plasminogen activator (rt-PA).
