Presentation and Outcomes of Pregnancy in Patients With Autoimmune Hepatitis

Autoimmune hepatitis (AIH) frequently affects women of childbearing age in whom the desire to have a family raises the question regarding the potential risks for the fetus and the mother. The information on AIH in pregnant patients is scarce.1 The aims of this study were (1) to identify the risk factors associated with flares in pregnant patients diagnosed with AIH, (2) to determine the course of AIH in patients with pregnancy-related flares, and (3) to describe the outcome of AIH diagnosed in the postpartum period.

We assessed in a western population the efficacy of a plasma-derived hepatitis B vaccine in relatives of highly infectious hepatitis B virus (HBV) carriers. A consecutive group of 103 HbsAg, anti-HBs and anti-HBc negative household relatives of 45 HBV-DNA positive chronic carriers received a 5 micrograms dose of plasma-derived vaccine at 0, 1, 2 and 12 months. Protective levels of immunity developed in 101 subjects (97.8%) 3 months after boosting. Low responders to the vaccine were mostly found among parents and spouses of carriers, whilst offspring and siblings were usually high responders. Abdominal scintigraphy with99mTc-pertechnetate, using a gamma-camera linked to a data processor, was done in 8 patients with acute massive intestinal hemorrhage and in 34 patients with chronic recurrent intestinal hemorrhage. Endoscopy and x-ray studies had failed in all patients to reveal a bleeding source. All but 3 patients underwent exploratory laparotomy, confirming the abnormal findings of scintigraphy. In all acutely bleeding patients, scintigraphy was positive. The bleeding was proven by laparotomy to be due to Meckel's diverticulum with gastric mucosa in 5, jejunal neurinoma in 1, and sigmoid colon carcinoma in 1 patient. “Preemptive” Live Donor Liver Transplantation for Fibrolamellar Hepatocellular Carcinoma: A Case Report

Fibrolamellar (FL) hepatocellular carcinoma (HCC) is a distinctive form of primary HCC that occurs principally in children and young adults. Although liver transplantation is not contraindicated for FL-HCC, noncirrhotic patients with large HCC tumors (including FL-HCCs) are not prioritized. Although hepatic resection is considered to be the primary treatment for FL-HCC, living donor liver transplantation is evolving into a potentially better alternative. Herein we have reported successful "preemptive" living donor liver transplantation for presumed recurrence of FL-HCC after an extended right hepatectomy with resection and synthetic graft replacement of the inferior vena cava.
