Involvement of SYCP2L and TDRD3 gene variants on ovarian reserve and reproductive outcomes: a cross-sectional study

2023

Objective: Single nucleotide variants have been implicated in the response to fertility treatment and a pharmacogenomic approach may help to customize therapy based on patient genome. We aimed to evaluate the effect, individual and combined, of SYCP2L (rs2153157:G>A) and TDRD3 (rs4886238:G>A) variants on ovarian reserve, response to controlled ovarian stimulation (COS) and reproductive outcomes of women undergoing in vitro fertilization (IVF) treatment. Methods: This cross-sectional study included 149 normoovulatory women undergoing IVF. Genotyping was performed using the TaqMan real-time polymerase chain reaction method. Clinical parameters and reproductive outcomes were compared acc…

Predictive Value of Basal Serum Progesterone for Successful IVF in Endometriosis Patients: The Need for a Personalized Approach

2022

The data regarding the role of progesterone (P4) in reproductive events of endometriosis patients are limited. This prospective study aimed to examine the predictive value of basal P4 serum levels for successful in vitro fertilization (IVF) in patients with primary infertility and endometriosis. The study included 73 patients divided according to endometriosis treatment (surgery vs. control—no treatment). The general data, basal hormonal status, and pregnancy rates were determined for every patient. Clinical pregnancy was achieved in 40.3% of patients, and more often in patients treated for endometriosis before IVF. The regression analysis showed that higher basal P4 serum levels were assoc…

Impact of Obesity on Sentinel Lymph Node Mapping in Patients with apparent Early-Stage Endometrial Cancer: The ObeLyX study

2022

Conflicting data exists on the impact of Body Mass Index (BMI) on sentinel lymph-node (SLN) detection. The primary study endpoint was to investigate the impact of obesity on overall detection rate, bilateral mapping, and mapping failure rate of SLN. In addition, we evaluated possible differences in terms of surgical management and "empty-packet dissection" rate among obese and non-obese patients.Multicenter, propensity-matched, retrospective study. Patients with apparent early-stage endometrial cancer were included. Study population was divided into women with BMI/≥ 30 (Group-1 and Group-2). To lower the selection bias, a propensity-matched analysis was performed. Matching was based on hist…

A large multicenter propensity match study of sentinel lymph node biopsy feasibility in endometrioid variants of endometrial cancer

2022

Introduction: Sentinel lymph node (SLN) biopsy algorithm has been routinely applied in all endometrial endometrioid tumors, however, no studies analyzed the feasibility of SLN mapping in endometrioid variants (EV), which included villoglandular, secretory, ciliated cell, mucinous, and squamous differentiation. This study aimed to demonstrate the feasibility of SLN biopsy in EV of EC. Materials and methods: All patients undergoing minimally invasive surgical treatment for early-stage EC were included in the study. Patients were divided into 2 study groups: Group 1 which included patients with EV, and Group 2 which included patients with typical endometrioid histology. A propensity match anal…

Cervical cancer and intraepithelial neoplasia

2012

Cervical cancer is the second most frequent malignancy in women. Infection with high-risk papillomaviruses causes cervical cancer and can be prevented by vaccination. Secondary prevention using cytology and/or human papillomavirus testing successfully decreases the incidence of cancer by detection of cervical intraepithelial neoplasia (CIN). Colposcopy is used to locate CIN and to assure preservation of healthy tissue when removing CIN. Diagnosis of CIN and cancer is based on histopathology. In invasive disease, imaging techniques such as MRI or PET-CT help to assess the locoregional extent of the disease. For staging, the FIGO and TNM system are applied. Laparoscopic staging, especially of…

Myomectomy in infertile women: More harm than good?

2023

Adhesion formation following gynecological surgery remains a challenge. The adoption of minimally invasive surgical approaches, such as conventional or robotic-assisted laparoscopy combined with meticulous microsurgical principles and the application of adhesion–reducing substances, is able to reduce the risk of de novo adhesion formation but do not eliminate it entirely. Myomectomy is the most adhesiogenic surgical procedure and postoperative adhesions can have a significant impact on the ability to conceive. Therefore, when surgery is performed as infertility treatment, attention should be paid to whether the benefits outweigh the risks. Among several factors, the size and the location of…

Human papillomavirus infection in couples undergoing in vitro fertilization procedures: impact on reproductive outcomes.

2011

A prospective study was performed to assess the relationship between human papillomavirus (HPV) infection in 199 infertile couples and outcome of assisted reproductive technologies (ARTs). A highly statistically significant correlation between pregnancy loss rate (proportion of pregnancies detected by β-hCG that did not progress beyond 20 weeks) and positive HPV DNA testing in the male partner of infertile couples, compared with HPV negatives, was observed (66.7% vs. 15%).

Management of Infertility: A Practical Approach

2022

Management of Infertility: A Practical Approach offers an accurate and complete reference for the management of infertility and a robust step-by-step guide for assisted reproduction technologies (ARTs), including how to plan, design and organize a clinical setting and laboratory. The book also provides an evidence-based, complete and practical description of the available methods for diagnosis and management of male and female infertility. This will be an ideal resource for researchers, students and clinicians who want to gain complete knowledge about both basic and advanced information surrounding the diagnosis and management of infertility and related disorders.

PCOS and infertility. Metformin administration and ovulation induction in patients with reproductive failures. Preliminary data

2014

The aim of this study is to investigate the effects of the metformin administration in patients with previous reproductive failures. Inclusion criteria: 13 patients with chronic anovulation and PCOS, age < 38 years old, sterility not less than 2 years, C.C. resistance, conception failure in previous cycles with only r-FSH and negativity to the multiple miscarriages tests, absence of others infertility factors. Metformin administration started 2 months before the ovulation induction. Alternatively a spontaneous menstruation it was induced by progestin. Ovulation was performed with r-FSH and patients were invited to have sexual intercourse during the 48h after the HCG trigger. Luteal support …

Uterine cervical stenosis: from classification to advances in management. Overcoming the obstacles to access the uterine cavity

2023

Background: To date hysteroscopy is the gold standard technique for the evaluation and management of intrauterine pathologies. The cervical canal represents the access route to the uterine cavity. The presence of cervical stenosis often makes entry into the uterine cavity difficult and occasionally impossible. Cervical stenosis has a multifactorial etiology. It is the result of adhesion processes that can lead to the narrowing or total obliteration of the cervical canal. Purpose: In this review, we summarize the scientific evidence about cervical stenosis, aiming to identify the best strategy to overcome this challenging condition. Methods: The literature review followed the scale for the q…
