Non-adherence to medical advice is a serious problem to patients, health policy and practitioners. This article outlines concepts of behavioral economics that might lead a patient to decide against the provider's recommendations and thus to be non-adherent. Especially the timing of pay-offs and dynamic inconsistency, their uncertainty and ambiguity aversion, loss-aversion and numerous heuristics like the peak-end-rule are discussed. The paper concludes with some hints on “libertarian” paternalism that may improve the situation.
Cost-efficacy of oral triptans in the treatment of acute migraine

SummaryThe objective of this study was to compare the efficiency of oral triptans currently used in the treatment of acute migraine in Spain. Using a decision analytic model, a cost-efficacy analysis was performed from the payer's perspective. Efficacy was assessed in terms of sustained pain-free patients, with data extracted from a recent meta-analysis of clinical trials. Chest-related and central nervous system-related adverse events were also considered. The recent joining of ten new member states to the European Union, eight of which are former communist countries, has reopened inherent tensions in current European Union (EU) policy-making on safety and health in the workplace.
