Neighborhood environment, social participation, and physical activity in older adults with lower limb osteoarthritis: A mediation analysis

Older adults with lower limb osteoarthritis (LLOA) are highly dependent on their physical and social environment for being physically active. Longitudinal data from 2286 older adults (M age = 73.8 years; 50.3% female) in six European countries were analyzed using cross-lagged Structural Equation Modeling (SEM) and multi-group SEM. In cross-sectional analyses, neighborhood resources were associated with physical activity (r = 0.26;p <.001) and social participation (r = 0.13;p =.003). Zur Uberprufung der diagnostischen Treffsicherheit wurden prospektiv 172 Kniegelenke mittels Magnetresonanztomographie (MRT) untersucht. Nach der MRT erfolgte bei allen Kniegelenken die arthroskopische Uberprufung der bildgebenden Diagnose. Rissbildungen des Innenmeniskus fanden sich bei 102 Patienten, am Ausenmeniskus wurde 29-mal ein Riss befundet. Unter der Annahme, dass die Arthroskopie der “golden standard” diagnostischer Masnahmen am Kniegelenk darstellt, ergab sich, dass falsch-positive MRT-Befunde am Innen- und Ausenmeniskus in jeweils 3 Fallen und falsch-negative MRT-Befunde am Innenmenikus bei 2 Kniegelenken und am Ausenmeniskus 4-mal erhoben wurden. In rheumatoid arthritis (RA) we examined the cruciate ligaments of 16 patients by the use of light- and transmission electron microscopy. Considerable differences between the anterior (ACL) and the posterior cruciate ligament (PCL) were observed. The ACL showed dramatic ultrastructural changes, against that the PCL was compact and well conserved. The preservation of PCL in joint surgery can morphologically be recommended. Perhaps the greater inflammatory process on the surface of ACL is responsible for the observed differences.

Lumbar sympathetic blocks (LSBs) are commonly performed to treat pain ailments in the lower limbs. LSBs involve injecting local anesthetic around the nerves. The study aimed at assessing the acute physiological effects of running a 65-km vs a 107- km mountain ultramarathon. Nineteen athletes (15 males and 4 females) from the shorter race and forty three athletes (26 males and 17 females) from the longer race were enrolled. Body weight, respiratory and lower limb strength were assessed before and after the race. Blood samples were obtained before, after and 24-h post-race. Body weight loss did not differ between races.
