Predicting Gleason Group Progression for Men on Prostate Cancer Active Surveillance: Role of a Negative Confirmatory Magnetic Resonance Imaging-Ultra…

2018

Active surveillance has gained acceptance as an alternative to definitive therapy in many men with prostate cancer. Confirmatory biopsies to assess the appropriateness of active surveillance are routinely performed and negative biopsies are regarded as a favorable prognostic indicator. We sought to determine the prognostic implications of negative multiparametric magnetic resonance imaging-transrectal ultrasound guided fusion biopsy consisting of extended sextant, systematic biopsy plus multiparametric magnetic resonance imaging guided targeted biopsy of suspicious lesions on magnetic resonance imaging. All patients referred with Gleason Grade Group 1 or 2 prostate cancer based on systematic…

Resistance of Cancer Stem Cells to Cell-Mediated Immune Responses

2015

In the past decades, the hierarchical organization of tumors, governed by Cancer Stem Cells (CSCs), have been reported with regard to several tumor types. Advances in sequencing technologies have demonstrated that diverse genetic CSCs subclones, derived from the branching evolution, compete with each other within the tumor mass, thereby contributing to the functional heterogeneity. It is becoming increasingly clear that epigenetic modifications and microenvironmental influences are important determinants of tumor fitness resulting in disease progression, recurrence and reduced patient survival. Therefore, more effective therapies will require gaining insights into the role of genetic and no…

Altered metabolism of gut microbiota contributes to chronic immune activation in HIV-infected individuals.

2015

Altered interplay between gut mucosa and microbiota during treated HIV infection may possibly contribute to increased bacterial translocation and chronic immune activation, both of which are predictors of morbidity and mortality. Although a dysbiotic gut microbiota has recently been reported in HIV + individuals, the metagenome gene pool associated with HIV infection remains unknown. The aim of this study is to characterize the functional gene content of gut microbiota in HIV + patients and to define the metabolic pathways of this bacterial community, which is potentially associated with immune dysfunction. We determined systemic markers of innate and adaptive immunity in a cohort of HIV-in…

Stanley Fahn Lecture 2005: The staging procedure for the inclusion body pathology associated with sporadic Parkinson's disease reconsidered.

2006

The synucleinopathy known as sporadic Parkinson's disease (PD) is a multisystem disorder that severely damages predisposed nerve cell types in circumscribed regions of the human nervous system. A recent staging procedure for the inclusion body pathology associated with PD proposes that, in the brain, the pathological process (formation of proteinaceous intraneuronal Lewy bodies and Lewy neurites) begins at two sites and continues in a topographically predictable sequence in six stages, during which components of the olfactory, autonomic, limbic, and somatomotor systems become progressively involved. In stages 1 to 2, the Lewy body pathology is confined to the medulla oblongata/pontine tegme…

Geldanamycin and its derivatives as Hsp90 inhibitors

2012

The Hsp90 molecule, one of the most abundant heat shock proteins in mammalian cells, maintains homeostasis and prevents stress-induced cellular damage. Hsp90 is expressed under normal conditions at a level of about 1-2 Percent of total proteins, while its expression increases 2-10 fold in cancer cells. The two main constitutively expressed isoforms of Hsp90 are known as Hsp90-alpha and Hsp90-beta, and their upregulation is associated with tumor progression, invasion and formation of metastases, as well as development of drug resistance. The Hsp90 is a key target for many newly established, potent anticancer agents containing Hsp90 N-terminal ATP binding inhibitors, such as geldanamycin, and…

Communication and ethical considerations for fertility preservation for patients with childhood, adolescent, and young adult cancer

2021

Patients with childhood, adolescent, and young adult cancer who will be treated with gonadotoxic therapies are at increased risk for infertility. Many patients and their families desire biological children but effective communication about treatment-related infertility risk and procedures for fertility preservation does not always happen. The PanCareLIFE Consortium and the International Late Effects of Childhood Cancer Guideline Harmonization Group reviewed the literature and developed a clinical practice guideline that provides recommendations for ongoing communication methods for fertility preservation for patients who were diagnosed with childhood, adolescent, and young adult cancer at a…

Macroelement Model for the Progressive-Collapse Analysis of Infilled Frames

2021

A new multistrut macromodel for the analysis of the progressive-collapse response of infilled reinforced concrete (RC) frames is presented in this paper. The model consists of three struts: two outer infinitely rigid and resistant struts and one inner fiber-section strut. The inclination of the struts as well as the stress-strain response are modulated by two parameters that are obtained by means of analytical correlations provided in the paper. The latter link the geometric and mechanical properties of an infilled frame to the geometric configuration and mechanical response of the equivalent strut model. This confers the model the capability to adapt to consider different collapse configur…

Association of redox and inflammation-related biomarkers with prognosis in IgA nephropathy: A prospective observational study.

2022

IgA nephropathy (IGAN) has a variable prognosis. Risk stratification tools are usually based on clinical parameters combined with histologic Oxford-MEST-C score. Circulating redox- and inflammation-related biomarkers may be related to histological changes in IGAN. Therefore, we studied the performance of these biomarkers in predicting the rate of GFR-loss in IGAN.This was an observational prospective study. Fifty-seven stable patients with IGAN were examined at baseline and after a mean observational time of 5.9 ± 1.1 years. The main outcome measure was eGFR-loss per year with predefined groups, stable (1.5 ml/min/1,73 mFifteen patients were in the progressive, 11 in the intermediate, and 3…
