It is generally assumed that drugs which induce relaxation of smooth muscles may be of clinical importance in some urological disorders; such drugs are indeed widely used, for example in the therapy of unstable bladders or to facilitate the passage of ureteral stones. Antispasmodic action may be classified in neurotropic and musculo-tropic action; the former acting on the autonomic nervous system and the latter directly on smooth muscle cells. Examples for the first type of action are anticholinergic drugs or alpha-adrenoceptor-antagonists, whereas papaverine is a classic drug with the second type of action.

ABSTRACT In 2004, 65 CTX-M-producing Escherichia coli isolates were collected from infected patients in four French hospitals. The bla CTX-M-15 genes were predominant. Pulsed-field gel electrophoresis highlighted a clonal propagation of CTX-M-15-producing strains belonging to phylogenetic group B2, notably in the community. The main risk factors for acquiring these isolates were urinary tract infections or the presence of a urinary catheter in diabetic or renal failure patients.

Objectives: Poultry have been suggested as a reservoir for fluoroquinolone-resistant extraintestinal pathogenic Escherichia coli (ExPEC). Our aim was to investigate whether genotypes associated with ciprofloxacin and multidrug resistance were shared among human and avian E. coli. Methods: We compared 277 human ExPEC isolates from urinary tract infection (UTI) and sepsis (142 susceptible and 135 ciprofloxacin resistant) and 101 avian isolates (68 susceptible and 33 ciprofloxacin resistant) by antimicrobial resistance phenotype, phylogenetic group and multilocus sequence type (ST). Continent anal urinary diversion is a therapeutic option in bladder exstrophy. We report our long-term results with the rectosigmoid pouch (Mainz pouch II), a modification of the classic ureterosigmoidostomy.A total of 38 children with a mean age of 5 years (range 0.5 to 17) underwent a Mainz pouch II procedure between 1991 and 2004. Most patients (33) had bladder exstrophy or incontinent epispadias. In 14 children (37%) urinary diversion was performed after failed primary reconstruction. In 6 children conversion was performed from an incontinent type of urinary diversion. Renal function, continence and metabolic changes were analyzed.
