Considering its position located anteriorly to the brainstem, the retrosellar area, in particular the upper clival region and the interpeduncular cistern, is one of the most difficult regions to surgically approach. Breakthrough cancer pain (BTcP) has been defined as a transitory increase in pain intensity on a baseline pain of moderate intensity in patients on analgesic treatment regularly administered. This review provides updated information about the use of opioids for the treatment of BTcP, with special emphasis on the use of new rapid onset opioids (ROOs). Due to its slow onset to effect oral opioids cannot be considered an efficacious treatment for BTcP. Sedation is regarded as a common side-effect of most H1-antihistamines. This view must be accepted, yet can hardly be assessed under treatment of allergic disorders. Since central sedative potency is hard to evaluate, different methods of measurement have been introduced in the four phases of clinical investigation. β-cyclodextrins (β-CDs) have been evaluated as chiral recognition agents for the enantioseparation of ephedrines by liquid chromatography (LC). The compounds tested were norephedrine, ephedrine, pseudoephedrine, N-methylephedrine and N-methylpseuoephedrine. Objective— To study the lactational and hormonal responses to nasal administration of thyrotrophin-releasing hormone (TRH) in puerperal women with inadequate lactation. Design— Prospective randomized double-blind placebo-controlled study. Subjects— 19 puerperal women with inadequate lactation (<50% of normal milk yield) on the 5th day postpartum. Interventions— 10 women were allocated to receive TRH administered by a nasal spray of 1 mg, four times daily, between suckling episodes, for 10 consecutive days starting on day 6 postpartum.
