Abdominal scintigraphy for diagnosis of intestinal bleeding

Abdominal scintigraphy with99mTc-pertechnetate, using a gamma-camera linked to a data processor, was done in 8 patients with acute massive intestinal hemorrhage and in 34 patients with chronic recurrent intestinal hemorrhage. Endoscopy and x-ray studies had failed in all patients to reveal a bleeding source. All but 3 patients underwent exploratory laparotomy, confirming the abnormal findings of scintigraphy. In all acutely bleeding patients, scintigraphy was positive. The bleeding was proven by laparotomy to be due to Meckel's diverticulum with gastric mucosa in 5, jejunal neurinoma in 1, and sigmoid colon carcinoma in 1 patient. Abstract Objective: To investigate whether dopexamine hydrochloride beneficially influences splanchnic perfusion and regulators of the macrocirculation and microcirculation in patients undergoing surgery of the abdominal aorta. Design: Prospective, blinded, and randomized study. Setting: University-affiliated hospital. Participants: Twenty patients undergoing elective abdominal aortic surgery. Background Fluid management guided by oesophageal Doppler monitor has been reported to improve perioperative outcome. Stroke volume variation (SVV) is considered a reliable clinical predictor of fluid responsiveness. Consequently, the aim of the present trial was to evaluate the accuracy of SVV determined by arterial pulse contour (APCO) analysis, using the FloTrac™/Vigileo™ system, to predict fluid responsiveness as measured by the oesophageal Doppler. Ziel unserer Studie war die Erfassung der Genauigkeit der praoperativen Diagnostik (rektal-digitale Untersuchung, Biopsie und transanale Endosonographie) bei durch lokale Exzision behandelten Rektumtumoren. 552 Patienten mit Rektumkarzinom, Adenom, Karzinoiden oder seltenen benignen Tumoren wurden lokal exzidiert. Die Ergebnisse der rektal-digitalen Untersuchung, der Biopsie und der transanalen Endosonographie wurden mit dem postoperativen pathohistologischen Befund verglichen. Die Therapie des Gallengangscarcinoms wird unter dem Eindruck der endoskopischen Behandlungsmoglichkeiten teilweise kontrovers diskutiert. Wenngleich ein Vergleich zwischen den Patienten die chirurgisch und denen die endoskopisch bzw. radiologisch interventionell behandelt wurden aufgrund der unterschiedlichen Zusammensetzung der verschiedenen Patientenkollektive nicht moglich ist, zeigen doch retrospektive Arbeiten, das ein wesentlicher Unterschied in der 30-Tage-Letalitat nicht besteht.
