Slow release oral morphine versus methadone for the treatment of opioid use disorder

2019

ObjectiveTo assess the efficacy of slow release oral morphine (SROM) as a treatment for opioid use disorder (OUD).DesignSystematic review and meta-analysis of randomised controlled trials (RCTs).Data sourcesThree electronic databases were searched through 1 May 2018: the Cochrane Central Register of Controlled Trials, MEDLINE and EMBASE. We also searched the following electronic registers for ongoing trials: ClinicalTrials.gov, WHO International Clinical Trials Registry Platform, Current Controlled Trials and the EU Clinical Trials Register.Eligibility criteria for selecting studiesWe included RCTs of all durations, assessing the effect of SROM on measures of treatment retention, heroin use…

Long term outcomes of pharmacological treatments for opioid dependence: does methadone still lead the pack?

2014

The aim of this review was to update and summarize the scientific knowledge on the long term outcomes of the different pharmacological treatment options for opioid dependence currently available and to provide a critical discussion on the different treatment options based on these results. We performed a literature search using the PubMed databases and the reference lists of the identified articles. Data from research show that the three pharmacological options reviewed are effective treatments for opioid dependence with positive long term outcomes. However, each one has its specific target population and setting. While methadone and buprenorphine are first line options, heroin-assisted tre…

Occurrence of illicit drugs in two wastewater treatment plants in the South of Italy

2017

In this study the occurrence and the behavior of illicit drugs and their metabolites have been investigated for two wastewater treatment plants (WWTPs) (namely, WWTP-1 and WWTP-2) located in Sicily (island of Italy). Samples were analyzed for methamphetamine, cocaine (COC), 3,4-methylenedioxymethamphetamine (MDMA), methadone (METH), 2-ethylidene-1,5-dimethyl-3,3-diphenylpyrrolidine (EDDP), 3,4-methylenedioxy amphetamine (MDA); 3,4-methylenedioxy ethylamphetamine (MDEA), 11-nor-9-carboxy-Δ9-tetrahydrocannabinol (THC-COOH) and Benzoylecgonine (BEG). The BEG, COC, MOR and THC-COOH were found at the highest concentration in both WWTPs. The Wastewater-based epidemiology calculation for BEG, COC…

Intravenous Use of Methadone: Efficacy and Safety

2013

Parenteral methadone could be helpful in specific conditions, particularly in unstable conditions of when the oral route is unavailable. Intravenous methadone should be the preferred parenteral route, due to concerns about the local toxicity of subcutaneous route. This chapter assesses aspects regarding the pharmacokinetic issues, the clinical use, the problems associated with the use of intravenous methadone for chronic pain, as well as the perioperative use of intravenous methadone.

Methadone versus buprenorphine: data detected from Caltanissetta SER.T

2020

In last years, heroin-addicted have exponentially increased: this has made it necessary to identify a pharmacological strategy as effective as possible. With this purpose, a statistical investigation was conducted in a sample of individuals, aged between 18 and 50. They were diagnosed and subjected to different treatments at Ser.T of Caltanissetta (Sicily-Italy) during the period 2013-2017. The analyzed patients were treated with three different pharmacological therapy: methadone 0.1%, methadone 0.5%, buprenorphine and suboxone. We obtained percentages of responders, low responders and non-responders patients from data processing, based on used therapy. Considering pharmacological responses…

Clinical management of drug-drug interactions in HCV therapy: Challenges and solutions.

2013

Contains fulltext : 118153.pdf (Publisher’s version ) (Open Access) Hepatitis C virus (HCV) infected patients often take multiple co-medications to treat adverse events related to HCV therapy, or to manage other co-morbidities. Drug-drug interactions associated with this polypharmacy are relatively new to the field of HCV pharmacotherapy. With the advent of the direct-acting antivirals telaprevir and boceprevir, which are both substrates and inhibitors of the cytochrome P450 (CYP) 3A iso-enzyme, knowledge and awareness of drug-drug interactions have become a cornerstone in the evaluation of patients starting and continuing HCV combination therapy. In our opinion, an overview of conducted dr…

Prolonged-release buprenorphine formulations: Perspectives for clinical practice

2020

International audience; Buprenorphine and methadone are the two main opioid agonist treatments approved for opioid use disorder. Buprenorphine is a partial agonist of the mu opioid receptors, which has been merely available through sublingual form until now. In practice, the use of buprenorphine is smoother than that of methadone, and it induces reduced risks of overdose. However, sublingual buprenorphine also exposes to risks (e.g., withdrawal, misuse) and constraints (e.g., daily intake). Three new galenic formulations of prolonged-release buprenorphine (PRB) are being commercialized and should allow some improvements in patients' comfort and safety. This narrative review aims to describe…

Supportive and Palliative care

2021

The definition of palliative and supportive care is often overwhelming and may be confused in common clinical practice. Supportive care is the multiprofessional attention to the individual’s overall physical, psychosocial, spiritual, and cultural needs and should be available at all stages of the illness. Palliative care shares the same objectives of supportive care, and some authors consider it in the advanced stages of disease. Actually it is important to carry out all these interventions from the earlier phases of disease to improve patient quality of life, to prevent therapy discontinuation for side effects, and hence to an optimization of outcomes. Quality of life is a heterogeneous co…

Methadone versus morphine for postoperative pain in patients undergoing surgery for gynecological cancer: A randomized controlled clinical trial.

2020

Abstract Study objective The aim of this study was to compare methadone and morphine for the management of postoperative. Design Open, controlled study. Setting Postoperative recovering area, ward. Patients Sixty-four patients, ASA I-III, undergoing gynecological surgery for cancer. Interventions Morphine or methadone 0.15 mg/kg given preoperatively. After operation an intravenous morphine or intravenous methadone infusion at doses of 12 mg/day was started. Measurements Pain intensity and opioid consumption. Main results Methadone infusion provided a better analgesia in comparison with morphine infusion on the second day. Opioid consumption was significantly lower in the methadone group. No…
