Zur therapie der �sophagusvarizenblutung in der Bundesrepublik Deutschland (West) ? Ergebnisse einer Urnfrage

An 1076 chirurgischen und internistischen Abteilung in der BRD (West) wurde eine Umfrage zur Behandlung der Osophagusvarizenblutung durchgefuhrt. Es uberwiegt die Akutsklerosierung bzw. die Ballonsondenbehandlung mit nachfolgender Sklerosierung bei der aktiven Blutung. Die abdominelle Sperroperation wird bei der konservativ nicht stillbaren Blutung noch vor dem portokavalen Notshunt bevorzugt. Betablocker werden vor allem in internistischen Abteilungen zur Rezidivprophylaxe eingesetzt. Erst nach mehrfacher Rezidivblutung trotz Sklerosierung sehen etwa die Halfte der Abteilungen eine Indikation zum elektiven Shunt. Magenentleerung nach Magenresektion mit und ohne erhaltene Duodenalpassage

Bei Hausschweinen wurden 2/3-Magenresektionen mit erhaltener sowie ausgeschalteter Duodenalpassage durchgefuhrt, die Magenentleerung szintigraphisch uber 4 h gemessen und einer Vergleichsgruppe nichtresezierter Tiere gegenubergestellt. Fur die halbfeste, mit 99mTc markierte Testmahlzeit fanden sich experimentell keine Hinweise fur eine verzogerte Magenentleerung nach Ausschaltung der Duodenalpassage durch Roux-Rekonstruktion. B-I- and B-II-Roux-Resektion differierten in der Magenentleerung nicht. Tierexperimentelle Untersuchungen zum gestielten D�nndarm-transplantat als partieller extrahepatischer Gallengangsersatz

Mit einem gestielten Dunndarmpatch konnte in Einzelfallen eine Gallengangsstriktur dauerhaft erweitert werden. Man beobachtet jedoch eine Dilatation des Patches, deren Ursache ebenso wie deren Folgen auf die Leberfunktion bislang ungeklart sind. Mit einem gestielten Dunndarmpatch ist tierexperimentell ein partieller extrahepatischer Gallengangsersatz moglich. Eine Erweiterung des Patches wird regelmasig beobachlet. Ursache hierfur ist weder eine Denervation des Patches noch der intraduktale Gallengangsdruck, sondern moglicherweise ein Zug am Gefasstiel. Die „stadiengerechte Therapie” der akuten Pankreatitis, die jedem Schweregrad der Erkrankung angemessen ist, besteht neben der konservativen Basistherapie und Intensivmedizin im richtigen Einsatz operativer Masnahmen. Voraussetzung dafur ist die fruhe Diagnose und Beurteilung des Schweregrades der akuten Pankreatitis. The mesothoracic wings of Afrotropical Scutelleridae (Hemiptera: Heteroptera): morphology and taxonomic significance

Mesothoracic wings have been examined and compared in 32 species representing 20 genera of Afrotropical Scutelleri- dae. Morphological characters of the corium are described. Three main types of mesothoracic wing venation patterns can be recognized. The taxonomic significance of forewi ng characters in Scutelleridae is briefly discussed.

ABSTRACT  Aim: As part of the Catania symposium on lung metastasectomy we reviewed our practice of computed tomography (CT)-guided percutaneous transthoracic needle biopsy of pulmonary metastatic lesions with particular emphasis on diagnostic accuracy and nature of complications lesions. Materials & methods: 25 patients with metastatic lesions of the lung have been evaluated between May 2010 and February 2014. Ischemic cardiac diseases are the most frequent and deleterious pathologies leading to important cardiovascular-related mortality worldwide. Abstract: Id1 promotes carcinogenesis and metastasis, and predicts prognosis of non-small cell lung cancer (NSCLC)-adenocarcionoma patients. We hypothesized that Id1 may play a critical role in lung cancer colonization of the liver by affecting both tumor cells and the microenvironment. Depleted levels of Id1 in LLC (Lewis lung carcinoma cells, LLC shId1) significantly reduced cell proliferation and migration in vitro. Genetic loss of Id1 in the host tissue (Id1(-/-) mice) impaired liver colonization and increased survival of Id1 animals. Technique and results of the retroperitoneoscopic adrenalectomy via a lumbar approach

Introduction: Since 1992, endoscopic techniques have been used increasingly in adrenal-gland surgery. In the present paper, the technique of the retroperitoneoscopic adrenalectomy via a lumbar approach is described. Methods: The patient is placed in a lateral decubitus position. In the first step, a dilatation trocar is introduced in the retroperitoneal space to create an artificial cavity. The dilatation trocar is replaced by a blocking trocar to close off the operating field. After insufflation of CO2, two additional trocars are introduced in the area of the conventional flank incision. Adrenalectomy is performed via these ports. Abstract Noradrenaline, 1 μm, induced a sustained contractile response in rat isolated aorta in the presence and in the absence of extracellular Ca2+. After depleting the noradrenaline-sensitive intracellular Ca2+ stores, an increase in the basal tone of the aorta was observed during the incubation period in the presence of Ca2+ and in the absence of the agonist. We have tested the possible pathways through which Ca2+ enters the cell to refill the previously depleted Ca2+ pools, a process that is accompanied by an increase in tension.
