The Surgery of Neglected Distal Humerus Fractures in Children and Adults

The restoration of function after a neglected distal humerus fracture presents a formidable challenge to the surgeon because of both the complexity of the regional anatomy and the proximity of numerous neurovascular structures. Inadequate or unstable fixation, a failure to reposition the articular fragments anatomically, prolonged post-operative immobilization, or the development of soft-tissue complications will result in substantial disability for the patient. Complications after injuries affecting a child’s elbow are common due to understimating the severity of the injury on the native X-rays where the growth plates and cartilage structures can not be seen.

Von Februar 197 bis Februar 1998 wurde bei 518 Patienten die postoperative Thromboseprophylaxe mit niedermolekularem Heparin anhand der Laborparameter TAT und D-Dimer dosisadaptiert vorgenommen. Es traten vier Thrombosen (1,6%) ohne embolische Komplikation auf. Die praoperativen TAT- und D-Dimer-Werte aller im Gesamtverlauf dosisangepasten lagen im Mittel signifikant hoher als die der ubrigen Patienten. 83% aller Adaptierungen erfolgten dabei zwischen dem vierten und siebten postoperativen Tag.

Die Stabilisierung proximaler extraartikularer Tibiafrakturen stellt unter Verwendung der gebrauchlichen Osteosyntheseverfahren unverandert ein Problem dar. Die Wahl des Operationsverfahrens hangt unter anderem von der Situation der oftmals erheblich kompromittierten frakturumgebenden Weichteile ab. An der proximalen Tibia besteht zusatzlich das Problem von Fehlstellungen der Fraktur. Dose-Adjusted Thrombosis Prophylaxis in Trauma Surgery According to Levels of D-Dimer

In 234 trauma surgery patients, thrombosis prophylaxis with Nadroparin-Calcium low-molecular-weight heparin (LMWH) was adjusted according to levels of D-Dimer. Basic prophylaxis was 2,850 IU per day. If D-Dimer concentrations rose above 2 mg/L after the fourth postoperative (p.o.) day, LMWH was administered twice a day. Color Doppler ultrasound was performed between the fifth and seventh p.o. days. Patients were divided into a high-risk (group 1: hip, femur, or knee replacement surgery, n=102) and a moderate-risk group (group 2: other surgery of the knee, tibia, fibula, or foot, n=132). The authors present a measurement of the t{bar t} production cross section using events with one charged lepton and jets from p{bar p} collisions at a center-of-mass energy of 1.96 TeV. A b-tagging algorithm based on the probability of displaced tracks coming from the event interaction vertex is applied to identify b quarks from top decay. Using 318 pb{sup -1} of data collected with the CDF II detector, they measure the t{bar t} production cross section in events with at least one restrictive (tight) b-tagged jet and obtain 8.9{sub -1.0}{sup +1.0}(stat.){sub -1.0}{sup +1.1}(syst.) pb. Bei unfallchirurgischen Patienten tritt in bis zu 40% eine tiefe Beinvenenthrombose auf. Das individuelle Risiko kann kaum kalkuliert werden. 518 unfallchirurgische Patienten mit Prophylaxe durch eine tagliche Einzeldosis eines niedermolekularen Heparins (NMH) wurden praoperativ und bis zu 10 Tage postoperativ in einer prospektiven Untersuchung beobachtet. In einer retrospektiven Studie wurden die Begleitverletzungen, Behandlungsmerkmale, Morbiditat, Mortalitat und das radiologische und funktionelle Endergebnis von 222 Patienten mit einer Beckenringverletzung vom Typ B oder vom Typ C nach der Klassifikation von M.Tile eruiert. 52 Patienten wiesen eine "Open-book-Verletzung" auf (Typ B1), 48 Patienten eine laterale Kompressionsverletzung (Typ B2/B3), 122 Patienten wiesen eine C-Verletzung auf. Ziel der vorliegenden Untersuchung ist die Evaluierung der Niederfeldmagnetresonanztomographie an einem offenen System unter Berucksichtigung des Erfahrungsstands des Untersuchers. Treatment of femoral fractures after hip prosthesis

Im Zeitraum von 1995 bis 1997 wurden in unserer Klinik 24 huftprothetische Operationen durchgefuhrt mit Implantation eines Prothesenspezialschaftes aufgrund erheblichen Knochenverlustes sowie aufgetretener Frakturen des proximalen Femurs. Bei sechs Patienten erfolgte ein primarer Prothesenwechsel, sechsmal lag bereits praoperativ eine Femurfraktur bzw. eine Fraktur der bisherigen femoralen Prothesenkomponente vor, bei funf Patienten war es im Rahmen der Wechseloperation zu Frakturen gekommen. In this article a new measuring system for biomechanical studies of various osteosynthesis procedures on the tibia is described. The paired tibias are osteotomized and embedded in PMMA in accordance with a randomization protocol. Testing is then carried out under nonaxial loading at 350 N, 600 N and 900 N, uniaxial bending applying a force of 12 Nm, and torsion at a force of 5 Nm in a universal pneumatic testing machine. Ultrasonic sensors pick up movement at the fracture gap under external loading. Testing of the measuring setup has been successful, and relevant results were obtained.

AbstractIn total, 90 gelatinous spheres, averaging one meter in diameter, have been recorded from ~ 1985 to 2019 from the NE Atlantic Ocean, including the Mediterranean Sea, using citizen science. More than 50% had a dark streak through center. They were recorded from the surface to ~ 60–70 m depth, mainly neutrally buoyant, in temperatures between 8 and 24°C. Lack of tissue samples has until now, prohibited confirmation of species. However, in 2019 scuba divers secured four tissue samples from the Norwegian coast.
