Die Lagerung von Transplantationslebern in kaltem Medium fuhrt zur Aktivierung der Glykolyse aufgrund des Sauerstoffmangels. Die glykolytische Energie ist moglicherweise entscheidend fur das Uberleben der Zelle, und Leberzellen sterben moglicherweise ab, sobald die Glykolyse aufgrund der Anhaufung von Endprodukten oder Mangel an Substrat (Glykogen) inhibiert wird. Bile Duct Warmer in Hepatic Cryosurgery—A Pig Liver Model

Freezing of the common bile duct resulted in injury, stenosis, or perforation of the bile duct in a dog model. Biliary cutaneous fistulas and bile leaks are reported as complications of hepatic cryosurgery in man. In an ex vivo pig liver model we compared freezing close to the bile duct with and without warming the bile duct with warmed saline solution via an inserted catheter ("bile duct warmer"). Abstract We aimed to assess the thermal profile and size of iceballs produced by Accuprobe cryoprobes in fresh porcine and human liver and human colorectal cancer liver metastases in vitro to allow better planning of cryosurgical treatment of liver metastases. Rat liver preservation by hypothermic oscillating liver perfusion compared to simple cold storage.

Rat livers were preserved hypothermically for 10 or 24 h in vitro as if for transplantation. Two methods of preservation were compared using physiological and biochemical parameters: simple storage and oscillating perfusion. By measuring the nucleotides after preservation the calculated energy charge was significantly higher after 10 and 24 h of oscillating perfusion compared to the simple storage group. Two different methods of liver preservation were compared : simple cold storage (CS) and hypothermic oxygenated perfusion extracorporal (HOPE). After 10 h of preservation (4°C) with modified UW solution reperfusion was performed by isolated liver perfusion for 90 minutes. Ischemia reperfusion injury (IRI) represents a major clinical problem in liver resection and transplantation. The present work investigated the potential of α-lipoic-acid (LA), a well established drug in the therapy of diabetes, to reduce IRI of the rat liver in vivo. Two groups were chosen: one group with injection of 500 μmol α-lipoic acid 15 min before clamping of the left and median liver lobes for 90 min, the other with injection of NaCl 0,9%. Reperfusion was carried out for 1 h and 7 days. The results showed that a-lipoic acid reduced IRI. Simple cold storage of livers for transplantation activates glycolysis due to lack of oxygen. Energy derived from glycolysis may be critical for cell survival and liver cell death may occur once glycolysis is inhibited in the liver due to accumulation of end products or lack of substrates (glycogen). A rat colon cancer cell line (CC531) was implanted into the livers of 85 WAG rats and in animals surviving the implantation the liver tumour was subsequently treated with sham laparotomy (n = 15), resection (n = 20), cryotherapy (n = 19) or laser therapy (n = 20). Local recurrence or persistence of the tumour in animals surviving the procedure was observed in 15 of 15 following sham laparotomy, in 0 of 15 following resection, in 3 of 17 following cryotherapy and in 10 of 14 following laser therapy. However, peritoneal disease and lung metastases were more frequent and extensive following resection as compared to cryotherapy or laser therapy.

An der Klinik und Poliklinik fur Allgemein- und Abdominalchirurgie der Johannes Gutenberg-Universitat Mainz wurden im Rahmen eines Programms zur systematischen klinikinternen Qualitatssicherung zwischen dem 1. 4. 1993 und 31. 12. 1995 3183 Patienten und deren perioperativer Verlauf erfast. Es zeigte sich eine signifikante Zunahme des Alters und der Morbiditat der operierten Patienten. Bei Zunahme der Notwendigkeit intensivmedizinischer Masnahmen blieben Morbiditat und Letalitat gleich.
