Targeted Therapies in Hepatocellular Carcinoma

2015

Hepatocellular carcinoma (HCC) is one of the main causes of death in patients affected by chronic liver diseases. The awareness of the biomolecular mechanisms underlying the complex carcinogenic process led to the development of targeted molecules, which are able to block this process. Sorafenib is a tyrosine-kinase inhibitor, and currently, it is the only drug approved by the US Food and Drug Administration (FDA) for the treatment of HCC. However, some studies have demonstrated the efficacy of single drug therapies or combination therapies. They showed an improvement with regard to overall survival, time to progression, and progression-free survival, although these therapies were not free …

Diabetes Mellitus and Ramadan: Physiopathological, Clinical and Therapeutic Aspects

2017

Fasting from dawn to sunset during Ramadan is one of the five pillars of Islam. A significant number of diabetic patients insist on fasting during Ramadan against the recommendations of their physicians. Fasting in diabetic subjects may be associated with increased risk of hypoglycemia and hyperglycemia, diabetic ketoacidosis, dehydration and thrombosis. Patients with uncontrolled diabetes mellitus are predisposed to major metabolic risks. Another problem is the reluctance of diabetic patients in taking their medications during the fast; therefore the timing and the dosage of anti-diabetic drugs must be adapted for each patient. It is important for diabetic patients who wish to fast during …

Direct-acting antivirals and visceral leishmaniasis: a case report

2019

Abstract Background Visceral leishmaniasis is a vector-borne parasitic disease caused by protozoa belonging to the genus Leishmania. The clinical presentation of visceral leishmaniasis strictly depends on the host immunocompetency, whereas depressive conditions of the immune system impair the capability to resolve the infection and allow reactivation from sites of latency of the parasite. Case presentation We describe a case of visceral leishmaniasis (VL) that occurred in a patient with chronic hepatitis C treated with direct-acting antiviral drugs (DAA). The hypothesized mechanism is the alteration of protective inflammation mechanisms secondary to DAA therapy. Downregulation of type II an…

Re-emergence of HIV Infection and Syphilis Among Men Who Have Sex with Men

2012

Sexual activity has been shown to be the primary mode of transmission for several important viral and bacterial infections among men who have sex with men (MSM) throughout the world and therefore sexually transmitted infections (STIs) are still one of major public health problem worldwide. Since the earliest days of the human immunodeficiency virus (HIV) epidemic MSM have been profoundly affected by the disease; by the time the first AIDS cases were reported in the early 1980s, HIV had already spread widely among homosexual and drug-user communities. In western Europe, has been estimated that HIV incidence peaked around 1983 among MSM and in 1987-88 among injecting drug users, with 120.000 …

Sirtuins and Aging: is there a Role for Resveratrol?

2016

Prolonged human life duration is consequently associated with a higher incidence of chronic diseases. Aging is a very complex process in which genetic, environmental and cellular pathways are involved. Along with aging, longevity has been linked with Sirtuins. Sirtuin enzymes are a family of highly conserved protein deacetylases that have been linked with calorie restriction and aging by modulating energy metabolism, genomic stability and stress resistance. Aim of this brief review is to describe Sirtuins’ influence on the conditions that worsen the physiological aging. We will also report the beneficial effects of the polyphenol resveratrol on these molecules and the possible therapeutical…

Molecular and pro-inflammatory aspects of COVID-19: The impact on cardiometabolic health

2022

Obesity, type 2 diabetes (T2DM), hypertension (HTN), and Cardiovascular Disease (CVD) often cluster together as "Cardiometabolic Disease" (CMD). Just under 50% of patients with CMD increased the risk of morbidity and mortality right from the beginning of the COVID-19 pandemic as it has been reported in most countries affected by the SARS-CoV2 virus. One of the pathophysiological hallmarks of COVID-19 is the overactivation of the immune system with a prominent IL-6 response, resulting in severe and systemic damage involving also cytokines such as IL2, IL4, IL8, IL10, and interferon-gamma were considered strong predictors of COVID-19 severity. Thus, in this mini-review, we try to describe the…

Evidence Based Prevention: un nuovo modello in Sanità Pubblica

2005

The concept of evidence-based prevention (EBP), was developed during the last decade as the counterpart, in Public Health, of Evidence-Based Medicine (EBM) in Clinical Medicine. The growing debate on EBP reflects the increasing recognition that successful evaluation of public health interventions will necessarily entail the use of research designs other than controlled trials as well as the use of various other types of evidence, often in combination. Evidence-based prevention, therefore, aims to implement those prevention strategies that have been found to be useful and effective through a formal evaluation based on scientific methodology. Prevalence of sexually transmitted infections and enteric protozoa among homosexual men in western Sicily (south Italy)

2012

Introduction. In recent years an increase in the number of sexu- ally transmitted infections (STIs) in men who have sex with men (MSM) has been reported in different industrialised countries. Because few epidemiological data on the STIs/MSM population in Sicily are available, a survey was conducted to assess the preva- lence of STIs/enteric protozoa and risky sexual behaviours among MSM in western Sicily. Methods. In 2010, 74 MSM with median age of 30 years old, were recruited via networks. All participants to the study were interviewed by anonymous self-administered questionnaire in order to collect social/demographic information, clinic data and STI-related risky sexual behaviours. Targeted Therapies in Hepatocellular Carcinoma

2014

Abstract: The onset of hepatocellular carcinoma (HCC) is related to the development of non-neoplastic liver disease, such as viral infections and cirrhosis. Even though patients with chronic liver diseases undergo clinical surveillance for early diagnosis of HCC, this cancer is often diagnosed in advanced stage. In this case locoregional treatment is not possible and systemic therapies are the best way to control it. Until now sorafenib, a Raf and multi-kinase inhibitor has been the best, choice to treat HCC systemically. It showed a survival benefit in multicenter phase III trials. However the proper patient setting to treat is not well defined, since the results in Child-Pugh B patients a…

The Dual Pandemics of COVID-19 and Obesity: Bidirectional Impact

2022

: The severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2), responsible for the COVID-19 pandemic, has been shown to disrupt many organ systems in the human body. Though several medical disorders have been affected by this infection, a few illnesses in addition may also play a role in determining the outcome of COVID-19. Obesity is one such disease which is not only affected by the occurrence of COVID-19 but can also result in a worse clinical outcome of COVID-19 infection. This manuscript summarizes the most recent evidence supporting the bidirectional impact of COVID-19 and obesity. It highlights how the presence of obesity can be detrimental to the outcome of COVID-19 in a given …

In a previous study we evaluated the microbiological quality of water of seven pools in the city of Palermo through evaluation of bacterial indicators of faecal contamination and of protozoa (Giardia and Cryptosporidium). In this study we also searched for the presence of fungi in two swimming pools of a public swimming facility in the same city. Samples were collected from both pools, their filtration systems and floor surfaces of the facility. Chemical-physical and microbiological examination of water of the two pools have shown that quality of water depends on the concentration of residual free chlorine and on the number of bathers in the pool. Analysis of electrolyte abnormalities and the mechanisms leading to arrhythmias in heart failure. A literature review.

2016

About 50% of deaths from heart failure (HF) are sudden, presumably referable to arrhythmias. Electrolyte and acid-base abnormalities are a frequent and potentially dangerous complication in HF patients. Their incidence is almost always correlated with the severity of cardiac dysfunction; furthermore leading to arrhythmias, these imbalances are associated with a poor prognosis. The frequency of ventricular ectopic beats and sudden cardiac death correlate with both plasma and whole body levels of potassium, especially in alkalemia. The early recognition of these alterations and the knowledge of the pathophysiological mechanisms are useful for the management of these HF patients.
