Multifocal Biliary Cystadenocarcinoma of the Liver: CT and Pathological Findings

2006

Biliary cystadenocarcinoma is a rare tumor that originates from the hepatobiliary epithelium. Although this tumor can affect any portion of the biliary tree, intrahepatic location is more common. It is usually a slow growing tumor and often asymptomatic until it reaches a considerable size. The lesion is most often solitary and large when discovered; multiple lesions or metastases within the liver are very rare. A 63-year-old man was referred to our institute for weight loss, abdominal discomfort, worsening bulky symptoms in the right upper abdominal quadrant, and an increase in serum aminotransferases that had been present for several months. Spiral CT of the abdomen demonstrated two lesi…

Techniques, Tricks, and Stratagems of Oral Cavity Computed Tomography and Magnetic Resonance Imaging

2022

The oral cavity constitutes a complex anatomical area that can be affected by many developmental, inflammatory, and tumoural diseases. MultiSlice Computed Tomography (MSCT) and Magnetic Resonance Imaging (MRI) currently represent the essential and complementary imaging techniques for detecting oral cavity abnormalities. Advanced MRI with diffusion-weighted imaging (DWI) and dynamic contrast-enhanced perfusion-weighted imaging (DCE-PWI) has recently increased the ability to characterise oral lesions and distinguish disease recurrences from post therapy changes. The analysis of the oral cavity area via imaging techniques is also complicated both by mutual close appositions of different mucosa…

Salvage Re-irradiation Options in Adult Medulloblastoma: A Case Report and Review of the Literature

2020

Background/aim Medulloblastoma is a rare tumor of adult age, while it occurs more frequently in children. Given the rarity, there is a lack of evidence for the treatment of recurrent disease. Few data are available about salvage re-irradiation collecting very heterogeneous series. Case report A 51-year-old male presented with headache, nausea, double vision, and gait disorders. A contrast-enhanced brain-MRI showed the presence of multifocal medulloblastoma. After surgery, adjuvant craniospinal radiotherapy was performed, chemotherapy was stopped due to toxicity. After 27 months, a new MRI and a Methionine-PET revealed a late pontocerebellar relapse; multidisciplinary board decided for a SBR…

Virtopsy and Living Individuals Evaluation Using Computed Tomography in Forensic Diagnostic Imaging

2019

The applications of forensic radiology involve both Virtopsy both studies on living people - to demonstrate bone age, search for foreign bodies, such as voluntary injection of drug ovules or surgical sponges accidentally forgotten, to assess gunshot wounds, to evaluate injuries by road accidents, and cases of violence or abuse (both in adults and in children). Computed tomography is the most used imaging tool used in forensic pathology and its indications are mainly focused on cases of unnatural deaths or when a crime is suspected. It is preferred over the standard autopsy in selected cases, such as in putrefied, carbonized or badly damaged bodies; or as a preliminary evaluation in mass dis…

Multimodal surgical and medical treatment for extensive rhinocerebral mucormycosis in an elderly diabetic patient: a case report and literature review

2014

Diabetes is a well-known risk factor for invasive mucormycosis with rhinocerebral involvement. Acute necrosis of the maxilla is seldom seen and extensive facial bone involvement is rare in patients with rhino-orbital-cerebral mucormycosis. An aggressive surgical approach combined with antifungal therapy is usually necessary. In this report, we describe the successful, personalized medical and surgical management of extensive periorbital mucormycosis in an elderly diabetic, HIV-negative woman. Mono- or combination therapy with liposomal amphotericin B (L-AmB) and posaconazole (PSO) and withheld debridement is discussed. MDCT and virtual angioscopy in spontaneous aortocaval fistula

2007

Aortocaval fistula is a rare, less than 1%, but life threatening complication, of abdominal aortic aneurysm. Mortality is high but prompt recognition of the fistula can reduce mortality rate. The multidetector row CT (MDCT) findings in a 69-year-old patient with a complex medical history characterized by previous episodes of myocardial ischemia, is reported. MDCT shows an early homogeneous enhancement of the inferior vena cava, slightly dilated at the liver level and markedly narrowed above the renal vein due to aneurysm compression. The patient underwent to emergency laparotomy but died during surgery for cardiac arrest. MDCT allows a prompt recognition of the fistula and different compute…

Correlation between ferrous ammonium sulfate concentration, sensitivity and stability of Fricke gel dosimeters exposed to clinical X-ray beams

2014

Abstract This work describes the characterization of various Fricke-Agarose-Xylenol gels (FXG) dosimeters using NMR relaxometry and MRI analysis. Using X-rays from a clinical linear accelerator (LINAC), the gels were irradiated in the dose range from 0 Gy to 20 Gy. The photon sensitivity of the FXGs was measured in terms of NMR relaxation rates; its dependence on radiation dose was determined as a function of ferrous ammonium sulfate contents (from 0.5 mM to 5 mM). Furthermore, the stability of the NMR signal was monitored over several days after irradiation. These measurements were aided by Magnetic Resonance Imaging (MRI) scans which allowed three-dimensional (3D) dose mapping. In order t…

The Videofluorographic Swallowing Study in Rheumatologic Diseases: A Comprehensive Review

2017

Autoimmune connective tissue diseases are a heterogeneous group of pathologies that affect about 10% of world population with chronic evolution in 20%–80%. Inflammation in autoimmune diseases may lead to serious damage to other organs including the gastrointestinal tract. Gastrointestinal tract involvement in these patients may also due to both a direct action of antibodies against organs and pharmacological therapies. Dysphagia is one of the most important symptom, and it is caused by failure of the swallowing function and may lead to aspiration pneumonia, malnutrition, dehydration, weight loss, and airway obstruction. The videofluorographic swallowing study is a key diagnostic tool in the…

Transitional cell carcinoma of endometrium: a case report of rare pure form

2018

Transitional cell carcinoma is a very rare histological subtype of endometrial cancer, with only 18 cases described in literature. Ovary represents the most common site where it accounts for 2% of all ovarian cancers. Due to the rarity of this disease, current management is controversial and includes various approaches, mostly described in the adjuvant setting, both radiotherapy and chemotherapy alone or in combination. Here, we report a case of a 65-year-old patient who underwent adjuvant vaginal brachytherapy for an endometrial transitional cell carcinoma. The patient was treated with radical total abdominal hysterectomy and bilateral salpingo-oophorectomy (TAH-BSO), without lymph-node sa…

La rappresentazione delle informazioni

2010

MR Imaging of Uterus Didelphys Transverse Vaginal Septum Causing Hematometrocolpos and Renal Agenesis

2002

The association of uterus didelphys and unilateral blind vagina with ipsilateral renal agenesis is an infrequent urogenital malformation distinguished by a lesion of the mesonephric duct. In most patients, symptoms are present at the beginning of the menstrual cycle because a transverse vaginal septum on one side obstructs the flow of menses, which consequently collect. Magnetic resonance imaging allows a precise delineation of this congenital abnormality, clearly demonstrating the transverse vaginal septum with hematometrocolpos and uterus didelphys. Extending the magnetic resonance study to the abdomen and employing coronal sections with a wide field of view is important to search for ass…

Umbilical venous catheters placement evaluation on frontal radiogram: application of a simplified flow-chart for radiology residents

2017

Background: Umbilical Venous Catheter (UVC) are commonly used in neonatal period; they can be not correctly positioned and could be associated with complications. The purpose of this article is to suggest a flow-chart to evaluate the placement of UVC, testing it in young radiologists-in-training. Method: We developed a simple flow-chart to asses, steps by step, UVC placement considering its course and tip location (ideally placed in the atriocaval junction). We tested the flow-chart impact asking to 20 residents to evaluate the placement of 10 UVC before and after they familiarized with the flow-chart and the anatomical findings of a newborn. Spontaneous Resorption of an Occipital Meningocele: Computed Tomography and Magnetic Resonance Imaging Evaluation

2016

Cranial meningocele is a very rare variant of encephalocele. Meningocele can be associated with other disorders and may cause complications. Therapy is usually based on surgical treatment. To our knowledge, we describe the first case of spontaneous resorption of an occipital meningocele in a full-term newborn boy. A full-term newborn was noted to have a large non-skin covered, semitransparent cystic lump in the occipital bone. He underwent computed tomography and a diagnosis of meningocele was proposed. After a few hours, the cystic lump spontaneously readsorbed. After 1 week the patient underwent magnetic resonance. Histology confirmed the diagnosis.

Computer tomography features of pulmonary vascular involvement in Behçet's disease

2019

Behçet’s disease is a chronic and multisystemic inflammatory vascular disease of unknown etiology, affecting vessels of all kinds and sizes. The involvement of the vascular system is observed in approximately 5-30% of patients. The most frequent vascular manifestations are venous thrombosis followed by arterial aneurysms and arterial thrombosis. The identification of vascular involvement is very important for prognosis, indeed the rupture of an arterial aneurysm represents the leading cause of sudden death in patients with Behçet’s disease. Computed tomography (CT) plays a crucial role in the identification and in the follow-up of vascular involvement. In this paper, we report the case of a…

Helical Tomotherapy® is a safe and feasible technique for total scalp irradiation

2017

Angiosarcoma of the scalp is a rare aggressive tumor that affects elderly patients. Chemoradiation is the treatment of choice for multicentric and extensive disease. The shape of the scalp represents a dosimetric challenge in terms of achieving a homogeneous concave dose distribution with coverage of the entire target volume and an acceptable organs-at-risk sparing. We report a case of an 81-year-old man with a multifocal angiosarcoma of the scalp treated with Helical TomoTherapy® (Accuray Inc., Sunnyvale, CA, USA) intensity modulated radiotherapy. This technique allows precise and daily verifiable coverage of the target keeping the dose to the organs at risk within the constraints.

Cumulative doses analysis in young trauma patients: a single-centre experience

2015

Multidetector computed tomography (MDCT) represents the main source of radiation expo- sure in trauma patients. The radiation exposure of young patients is a matter of considerable medical concern due to possible long-term effects. Multiple MDCT stud- ies have been observed in the young trauma population with an increase in radiation exposure. We have identi- fied 249 young adult patients (178 men and 71 women; age range 14-40 years) who had received more than one MDCT study between June 2010 and June 2014. According to the International Commission on Radio- logical Protection publication, we have calculated the cumulative organ dose tissue-weighting factors by using CT-EXPO software ® . We…

SEMI-AUTOMATIC VOLUMETRIC SEGMENTATION OF THE UPPER AIRWAYS IN PATIENTS WITH PIERRE ROBIN SEQUENCE

2014

Pierre Robin malformation is a rare craniofacial dysmorphism whose pathogenesis is multifactorial. Although there is some agreement in non-invasive treatment in less severe cases, the dispute is still open on cases with severe respiratory impairment. We present a semi-automatic novel diagnostic tool for calculating upper airway volume, in order to eventually address surgery in patients with Pierre Robin Sequence (PRS). Multidetector CT datasets of two patients and two controls were tested to assess the proposed method for ROI segmentation, upper airway volume computation and three-dimensional reconstructions. The experimental results show an irregular pattern and a severely reduced cross-s…

STUDY OF CAPILLARY ABSORPTION KINETICS BY X-RAY CT IMAGING TECHNIQUES: A SURVEY ON SEDIMENTARY ROCKS OF SICILY

2008

Sedimentary rocks are natural porous materials with a great percent of microscopic interconnected pores: they contain fluids, permitting their movement on macroscopic scale. Generally, these rocks present porosity higher then metamorphic rocks. Under certain points of view, this feature represents an advantage; on the other hand, this can constitute an obstacle for cultural heritage applications, because the porosity grade can lead to a deterioration of the lapideous monument for water capillary absorption. In this paper, CT (Computerized Tomography) image techniques are applied to capillary absorption kinetics in sedimentary rocks utilized for the Greek temples as well as baroc monuments, …

Presacral Myelolipoma

2014

Many reports have described adrenal myelolipomas but there have been only a few reports of extra-adrenal myelolipomas. We describe a 74-year-old woman who came to our observation for MRI of the lumbar spine for typical lumbar back pain. In addition to signs of mild scoliosis and spondylo disc arthrosis, MR imaging revealed a presacral mass showing a heterogeneously high signal in all pulse sequences and almost completely suppressed on inversion recovery sequences for fat tissue. CT imaging confirmed the fatty nature of the lesion and no signs of bone involvement. These findings were most consistent with a diagnosis of a rare presacral myelolipoma as confirmed at histopathologic analysis. T…

Hypo-fractionated stereotactic radiation therapy for lung malignancies by means of helical tomotherapy: report of feasibility by a single-center expe…

2018

Background: Several experiences in the literature report SBRT as an effective treatment option for medically inoperable early stage non-small cell lung cancer (NSCLC) and oligometastatic disease. The optimal fractionation schedules and total dose remain controversial. In this study, we evaluated the safety in terms of toxicity and efficacy of using of 8–10 fractions schedules with Helical Tomotherapy (HT) for primary and metastatic lung lesions. Methods: Between March 2014 and May 2016, a total of 39 patients (median age 72 years, range 26–91) were treated with HT-SBRT for malignant lung lesions: 22 patients with early stage NSCLC, 17 with oligometastases. Reversible effect of magnetic fields on human lymphocyte activation patterns: different sensitivity of naive and memory lymphocyte subsets.

2009

The aim of this study was to investigate the influence of 50 Hz magnetic or static magnetic fields of 0.5 mT on subsets of human CD4(+) T cells in terms of cytokine release/content, cell proliferation and intracellular free calcium concentration. CD4(+) T cells can be divided into different subsets on the basis of surface marker expression, such as CD45, and T cells can be divided into naive (CD45RA(+)) and memory (CD45RA(-)) cells. In this study, the effects of magnetic fields after 24 and 48 h of cell culture were analyzed. We found that the CD4(+)CD45RA(-) T subset were more sensitive after 2 h of exposure. Decreases in the release/content of IFN-gamma, in cell proliferation and in intra…

Computed tomography of bowel obstruction: Tricks of the trade

2015

Bowel obstruction (BO) is a frequent cause of hospitalization and surgical consultation for acute abdominal pain. It is usually suspected at physical examination, but clinical and laboratory data are often nonspecific. Thus, computed tomography plays a crucial role in a correct diagnosis of BO. Indeed, computed tomography can confirm a diagnosis of BO, and identify the location and cause of the obstruction. In this review, the computed tomography appearances of common and uncommon causes of BO and pseudo-obstruction are reviewed.

Imaging Evaluation of Facial Complex Strut Fractures

2012

High-resolution multidetector computed tomography with multiplanar reformations and 3-D postprocessing often provides the detail necessary for preoperative assessment of facial injuries. Maxillofacial fractures are classified in the following manner: upper face fractures, midface fractures (the most frequent), Le Fort fractures, and lower face or mandible fractures. The facial skeleton is a framework of vertical and horizontal buttresses that ensures a better resistance to trauma, but serves also as reference for maxillofacial surgery to restore facial size and shape. Radiologists should know how to diagnose and report the main types of facial fracture.

Cone-beam computed tomography in lung stereotactic ablative radiation therapy: predictive parameters of early response

2016

To analyze lung lesion volume variations by contouring on cone-beam CT (CBCT) images to evaluate the early predictive parameters of stereotactic ablative radiation therapy (SABR) treatment response. The prescribed dose of SABR was varied according to the tumour site (central or peripheral) and maximum diameter of the lesions by using a strategy of risk-adapted dose prescription with a dose range between 48 and 70 Gy in 3-10 consecutive fractions. For the purpose of the analysis, the gross tumour volume (GTV) was recontoured for each patient at first and last CBCT using two lung levels/windows: (a) -600/1000 HU and (b) -1000/250 HU. Univariate analysis was performed to evaluate a correlation …

Septate uterus: nosographic overview and endoscopic treatment

2014

To comment on the prevalence, diagnosis, and treatment of the septate uterus, with special reference to hysteroscopic metroplasty and its effect on reproductive outcome, we searched publications in PubMed and Embase. Original articles, meta-analysis, reviews, and opinion articles were selected. The studies suggest that the prevalence of the septate uterus is increased in women with repeated pregnancy loss and infertility. Reliable diagnosis depends on accurate assessment of the uterine fundal contour and uterine cavity by means of magnetic resonance and three-dimensional ultrasound. Pertinent published data comparing pregnancy outcome before and after hysteroscopic metroplasty indicated a m…

Ozone Infiltration for Osteonecrosis of the Jaw Therapy: A Case Series

2022

Medication-related osteonecrosis of the jaw (MRONJ) is a serious adverse reaction, mainly due to bone-modifying agents (BMA), and it is a potentially painful and debilitating condition. To date, the literature has reported a 90% rate of successful outcomes for MRONJ patients undergoing surgical treatment. Particularly for patients with advanced disease stages who are unsuitable for surgery, prolonged medical treatment is required, with a consequent risk of the overuse of antibiotics and antibiotic resistance. The aim of this study is to evaluate the efficiency and safety of ozone, via oral mucosal infiltrations, in seven cancer patients with MRONJ, who are not eligible for surgery. The prot…

Gastrointestinal lymphomas represent 5–20% of extranodal lymphomas and mainly occur in the stomach and small intestine. Clinical findings are not specific, thus often determining a delay in the diagnosis. Imaging features at conventional and cross-sectional imaging must be known by the radiologist since he/she plays a pivotal role in the diagnosis and disease assessment, thus assisting in the choice of the optimal treatment to patients. This review focuses on the wide variety of imaging presentation of esophageal, gastric, and small and large bowel lymphoma presenting their main imaging appearances at conventional and cross-sectional imaging, mainly focusing on computed tomography and magne…

Videofluorography and MDCT findings in a case of pharyngeal perforation after anterior cervical spinal surgery with fusion (ACSF)

2010

Purpose Anterior cervical spinal surgery with fusion (ACSF) is the most widely used technique in the management of cervical spondylosis or disc herniation. A case of pharyngeal perforation after ACSF demonstrated by videofluorography and MDCT is described. Methods A 39-year-old man, recently undergone to ACSF for interbody C3-C5 fusion with anterior cervical plate and screws, was submitted to a pharingoesophageal videofluorography and 64 row-MDCT of the neck before and after oral administration of iodine contrast medium for persistent high dysphagia. Results In the plain radiograph a fractured screw was detected, anterolaterally displaced on the right to the cervical spine, outside the phar…

Interventistica eco-guidata per l’articolazione d’anca

2013

The purpose of this work is to review the main applications of ultrasound-guided hip intervention al procedures describing the indications and methodology. Ultrasound is a method considered safe for patients as it has not been shown a possible damage due to ultrasound energy transmitted towards the patient's tissues as opposed to methods that make use of ionizing radiation such as conventional radiography and computed tomography. In addition ultrasound is quick and effective in guiding the needle to the desired location avoiding vascular and/or nerve structures thus enabling greater security compared to the techniques performed in interventional fluoroscopy which also have the disadvantage of a large delivery of ionizing radiation for both patient and operator. Several authors have described the main techniques of ultrasound-guided hip intervention and its various applications that can be both diagnostic (as in the case of joint fluid aspiration to assess the presence or absence of septic arthritis) and therapeutic (as in the case of drugs inoculations or evacuation of exceeding intra-articular fluid).

Multifocal biliary cystadenocarcinoma of the liver: CT and pathologic findings.

Spiral CT of the abdomen demonstrated two lesio…

Integrated techniques to evaluate the features of sedimentary rocks of archaeological areas of sicily.

2004

Sicily includes a great variety of lithologies, giving a high complexity to the geologic landscape. Their prevalent lithology is sedimentary. It is well known that rocks of sedimentary origin, compared with metamorphic and volcanic deposits, can be relatively soft and hence fairly easy to model. Nevertheless, this workability advantage is a drawback for Cultural Heritage applications. In fact, these materials show a high porosity, with pore-size distributions that lead to deterioration through absorption of water. In this paper, several sedimentary rocks used in historical Cultural Heritage items of Sicily, from "Magna Graecia" to nowadays, are classified for mineralogical features, chemica…

Rarely a blowout orbital fracture is related to sneezing in patients with no history of trauma or sinus surgery. A case of barotraumatic, pure and isolated blowout fracture of the right orbit after sneezing diagnosed by CBCT is reported. A forty-year-old man complained sudden right diplopia after vigorous sneezing. On MRI a reduced size and an altered shape of the right maxillary sinus, a slightly increased vertical diameter of the right orbit, a round shape of the inferior rectus muscle on coronal sections, and a right maxillary haemosinus were observed. A strong peripheral enhancement of the maxillary sinus wall was evident. One week after the patient was submitted to a maxillofacial CBCT…
